**UNIT PROFILE**:
The Concurrent Disorders Program is a newly developed service that will provide care to persons aged 16 and older presenting with severe and persistent mental illness and active substance abuse disorder/dependency. An integrated care model ensures a seamless integration of psychiatric and substance abuse interventions. The Capacity Building Team is a mobile team that utilizes knowledge translation strategies to develop system wide capacity across SJHH in Concurrent Disorders through the provision of consults, mentoring and education.

The primary responsibility of the Mental Health Worker will be to provide consults to clients who have complex addiction issues and Concurrent Disorder within the hospital and community for the purpose. The Mental Health worker works in collaboration with other members of the Concurrent Disorder Capacity Building Team to lead capacity building through knowledge exchange specific to concurrent disorders/addictions and leadership in building the organization’s capacity for delivering program-wide evidence-based harm reduction strategies (ie. tobacco withdrawal management, smoking cessation, comfort and prevention).

**QUALIFICATIONS**:

- Degree/Diploma in Nursing (RN) or Degree in Social Work (BSW or MSW) or Occupational Therapy **OR **current registration in good standing with the College of Psychotherapists with a Degree in a health related discipline
- Current registration in good standing with a related regulated health professional College
- CPI-Crisis Prevention Training (or successful completion as part of clinical orientation) is required
- Current First Aid/CPR certification
- Valid driver’s license and reliable transportation to be used in course of employment
- Minimum of 5 years recent related clinical experience in concurrent disorders/Addictions
- One year recent related clinical experience in the provision of adult education
- Demonstrated knowledge of individual counseling and group skills (ie. DBT, MI, IPT, CBT, ACT)

Concurrent Disorders-specific Qualifications:

- Completion of a certificate program in addictions/concurrent disorders from an accredited educational institution preferred
- Completion of the Certificate Program in Intensive Tobacco Cessation Counselling (TEACH) preferred
- Demonstrated knowledge of the stages of change model and ability to utilize stage specific interventions
- Demonstrated competence in the facilitation of addictions/concurrent disorder groups (relapse prevention)
- Demonstration experience working with a harm reduction model including the provision of Naloxone
- Demonstrated competence in conducting the screening, assessments of patients with addiction and concurrent disorders including the identification of risk factors and symptoms of withdrawal
- Demonstrated ability to administer addiction specific screening tools GAIN Q3 MI, Clinical Opiate Withdrawal Scale (COWS), The Clinical Institute Withdrawal Assessment for Alcohol (CIWA) - Ability to provide clinical consultation through the demonstration of therapeutic techniques and interventions specific to concurrent disorders/addictions
- Demonstrated skill in the incorporation of evidence based practice and relevant referrals into

addiction/concurrent disorder specific treatment plans
- Demonstrated ability to provide clinical support to patients with complex addiction/medical issues transitioning from hospital to community
- Demonstrated knowledge of and ability to access formal and informal community services and resources
- Demonstrated track record of providing leadership in building the organization’s capacity for delivering a hospital wide evidence-based concurrent disorder strategy
- Experience in leading case formulation, case conferences and providing clinical supervision

**HOURS OF WORK**:
Monday to Friday 10 hour shifts; days and holidays
