Quality Improvement Decision Support Specialist

The St. Michael’s Hospital Academic Family Health Team (SMHAFHT) is currently looking for a **Quality Improvement (QI) Specialist **to join our team. The SMHAFHT serves an inner-city population in downtown Toronto and core teaching site for the Department of Family and Community Medicine at the University of Toronto. In response to the unique needs of the communities we serve, our physicians and staff have developed clinical expertise in a number of areas including HIV primary care, addiction medicine, the care of people who are homeless or under-housed, people who self-identify as transgender, the poor, elderly, home-bound patients, young children, adolescents, pregnant women, and new immigrants. Quality improvement, social determinants of health and interprofessional education are among many activities that are a focus of our department and contribute to our mandate to provide excellent interprofessional care within an academic environment.
**Your opportunity**:
The SMHAFHT has developed streamlined pathways for new patient attachment (NEWR pathway) to prioritize those who are unattached to primary care, reside in the local community and are from equity-deserving populations.
We intend to use a Quality Improvement evaluation framework, such that through 2-3month PDSA cycling, we optimize the impact of SMHAFHT’s new patient pathways in providing equitable access, patient-centeredness in the on-boarding process, and optimize interprofessional provider resource utilization and provider well-being.
Working collaboratively with the Strategic Unattached New Patient Working Group and FHT staff, the **QI Specialist** will inform, implement and facilitate quality improvement related activities to enhanced patient attachment processes in the SMHAFHT, including the NEWR Pathway.
- Reporting to the Strategic Unattached New Patient Working Group and fostering positive relationships with clinical and community stakeholders by informing, tracking and collecting information on project progress
- Supporting quality improvement initiatives by conducting systematic literature reviews, developing process maps, designing end-user satisfaction surveys, capturing and analyzing data from various sources, including Telus Health Practice Solutions (EMR), and preparing draft statistical reports for QI research papers
- Meet with the Strategic Unattached New Patient Working Group team on a monthly basis to review metrics collected to date and to understand further opportunities for improvement and generate change ideas for the NEWR pathway to optimize equitable access, patient-centeredness and interprofessional provider resource utilization and well-being.
- Drafting summaries of the project results and formatting data collected for presentation to the Strategic Unattached New Patient Working Group, workshops for dissemination and the final New Patient Attachment Toolkit publication.
**Responsibilities**
- Leads quality improvement analysis, action plans and reporting in collaboration with the Strategic Unattached New Patient Working Group
- Leads implementation of QI project plans at all 5 Family Health Team sites;
- Reports on progress monthly to Strategic Unattached New Patient Working Group
- and to project sponsors, site and network committees and senior leadership as needed, which will include presentations at senior level committees and tables;
- Facilitates quality improvement planning sessions with leadership and program team;
- Provides analysis on performance and recommendations for corrective actions;
- Collaborates with support services to provide value to stakeholders related to the QI plan for new patient attachment;
- Supports site specific activity related to the NEWR Pathway, including facilitation of meetings, updating improvement dashboards and presentations at each FHT site;
- Works with clinical experts to champion change initiatives through the QI process;
- Uses innovative approaches to display data to prompt change
- Manages, coordinates and facilitates the AFP New Patient Recruitment quality/process improvement project, leading to enhancing the FHT’s quality of care, patient experience and resource utilization
- Utilizes advanced process redesign tools, quality improvement methodology/tools and change management expertise to assess current state of complex processes and facilitate the development of the ideal future state;
- Communicates regularly with project sponsors, e.g., Strategic Unattached New Patient Working Group, Exec Team, Quality Steering Committee etc, regarding project status/emerging challenges and works collectively to ensure improvement project success.
