**_About Pregnancy Pathways:_**

Pregnancy Pathways began as a collaboration of some 25 organizations that came together to address the housing and support needs of pregnant homeless women in Edmonton. Radius Community Health & Healing, which runs inner-city health and wellness programs, operates the Pregnancy Pathways program that includes a dedicated residence for 12 women with on-site support services. In addition to experiencing homelessness, program participants may face challenges with trauma, addictions, and mental health issues, require assistance to access prenatal care, and need support to be empowered to make decisions about the care of their infants.

**_ Job Description:_**

The Community Integration Support Worker plays a crucial role in supporting mothers within the program by guiding them in connecting to essential resources that foster the well-being and development of their children. This role involves creating a non-judgmental, supportive environment for mothers raising children often with Fetal Alcohol Spectrum Disorder (FASD) and similar circumstances, by understanding the unique challenges they face. The Community Integration Support Worker is responsible for facilitating connections to resources and services that can help mothers overcome obstacles and ensure their children thrive. Many mothers face challenges in navigating the complex system of resources available within Edmonton, and the difficulty in finding the right support can often feel overwhelming and discouraging. That is why it is crucial to have someone to help navigate accessing these resources.

In addition, the role focuses on empowering these women by setting them up with long-term supports, ensuring that once they graduate from the program, they are equipped to continue raising their children with access to the appropriate services and ongoing assistance.

**_ Key Responsibilities:_**
- Develop and cultivate community partnerships to facilitate the integration of both in-house and community-based programming.
- Identify opportunities for healthy growth and provide non-judgmental guidance to address participants' concerns.
- Collaborate as an active member of a strong team to offer ongoing support to participants within the program as needed.
- Implement harm reduction strategies and risk mitigation approaches to ensure the well-being of program participants.
- Provide prenatal education and support to individuals, promoting health and wellness throughout pregnancy.
- Assist participants with community involvement, including supporting appointments and facilitating recreational activities.

**_ Qualifications:_**
- A Community Studies Diploma or Degree, or a Child-Focused Diploma or Degree (e.g., Early Childhood Development, Child and Youth Care, Prenatal Education).
- Knowledge of Fetal Alcohol Spectrum Disorder (FASD) and an understanding of Indigenous practices and beliefs.
- Demonstrated experience in Trauma-Informed Care or person-centered care approaches
- A valid driver’s license and proper vehicle registration.
