Community Living Support Worker

**Job Summary**
The Community Living Support Worker ensures the well-being of clients and promotes their ongoing well-being and development. This role assists clients in their physical, social, emotional, and daily life skills development and increases their independence and ability to function within the community: provides support to clients including crisis intervention, behavior management, life skills training, and employment needs, whether on a one-to-one basis or in a group environment, facilitates, implements and evaluates recreational, social and educational activities for clients served by the organization; strives to create a fun and enjoyable atmosphere to encourage client participation in programs.
**Duties**
1. Facilitates case planning with clients, their families, and healthcare providers to determine programs that meet clients' mental, physical, recreational, social, and educational needs. Supports clients in their own homes, in programs, and in the community to allow them to function more safely and independently. Assists case management team in identifying potential problems and provides input to counselors and other professionals regarding the development and intervention of client program plans.
2. Provides supportive counseling using PSR principles, motivational interviewing, demonstrations, modeling, and behavior management to clients, their families, and/or personal networks to assist with the development of physical, social, emotional, and life skills such as maintaining personal hygiene, housekeeping, meal planning and preparation, meeting financial obligations, making/keeping appointments, and interpersonal skills. Accompanies clients to medical appointments to provide support and consistency when navigating day-to-day management of illness.
3. Monitors clients and recognizes potential crisis or emergencies, analyzes, and develops strategies to deal with such situations to ensure no harm to the client, staff, and/or the public. Reports medical, behavioral, and other problems, as required.
4. Identifies available social, economic, recreational, and educational services in the community that will meet clients’ needs; participates in group planning and helps develop individual activity plans that focus on the special needs of each client. Provides clients with related information.
5. Maintains written records and documentation regarding client activities and progress, for example: care plans, progress reports, statistics, and daily logs; ensures all required documentation and reports are up to date and accurate.
6. Facilities clients’ physical, recreational, and employment needs by performing duties such as arranging for clients’ transportation to programs/services, assisting clients to attend appointments, and implementing programs that meet the client's needs.
7. Maintains liaison with other agencies, professionals, government officials, and the community; monitors progress and well-being of clients and assists the case management team in evaluating the effectiveness of programs, counseling**,** and support.
8. Monitors clients and provides medical reminders; discusses’ the importance of medication and how it contributes to wellness along with other strategies. Reports to the clinical body when concerned about the status of a client’s medication regime.
9. Advocates for clients by problem-solving in areas such as property owner disputes, financial assistance, assessing community resources, and obtaining volunteer or employment placements.
10. Provides support to clients in managing their finances by conducting transactions related to petty cash, client accounts, collecting rent and money for personal and making deposits or withdrawals.
11. Ensures that all requirements are maintained to provide safe transportation, proper driver’s license, car maintenance, etc. Maintains up-to-date knowledge of building fire regulations and procedures and other emergency procedures; ensures all health and safety standards are maintained.
12. Performs other duties, as assigned.
**Qualifications/Requirements**:
1. Post-Secondary Diploma in a related discipline - Community Social Services
2. Must have a current Restricted Class 4 driver’s license
3. Hold a valid first aid and C.P.R. certificate
4. Vulnerable population’s criminal record clearance
5. Must have own vehicle with business coverage insurance
6. Food-safe certification is an asset
7. Must be able to work independently
8. Work in partnership with local agencies including Tri-Cities Mental Health
9. Ensure that all appropriate paperwork, record keeping, and reports (oral and written) are completed as required
10. Must have the ability to work with co-workers in a team environment
**Minimum Experience**:
1. 2 years related experience in the practice of recovery and psychosocial rehabilitation
2. Experience working with individuals with a diagnosis of co-occurring disorders
