Working under the direction of the Coordinator of Programs and Director of Services, the **Team Lead,** using a harm reduction approach is responsible for providing a broad spectrum of service to meet the needs of the program constituents via direct and indirect contact. The **Team Lead** will implement strategies based on situational evaluations, needs assessments, and the formulation of remedial strategies to meet the program needs in an effective and respectful manner. The **Team Lead** in collaboration with the Coordinator of Programs will ensure appropriate mental health, case management services, resource referrals, and crisis interventions are delivered to support constituents along their journey. The **Team Lead** will assist ICSAs (intensive community support advocates) to provide direct case management supports and assessments for constituents as needed, serves as a liaison and collaborates with community agencies/systems, maintain effective working relationships, and a working knowledge of housing first options. The **Team Lead** requires the ability and skills to work with constituents who may be oppositional and/or volatile, while at all times maintaining composure and behave in a professional and dignified manner. The **Team Lead** will ensure that all activities are undertaken in a manner that is consistent with and complementary to the Vision Statement, the Principles and Values, and the policies and procedures of the **Aboriginal Health & Wellness Centre**.

**DUTIES AND RESPONSIBILITIES**
- In collaboration with the Coordinator of Programs ensure that frontline staff follow-through on expectations, display and model expected behaviour, maintaining open lines of communication and being clear about roles and relationships
- Support staff to adhere to Housing First and Harm Reduction philosophy/ principles
- In collaboration with the Coordinator of Programs and the Director of Services provide leadership and guidance to staff on clinical issues, and record confidential information as required (PHIA, FIPPA) etc.
- Ensure that the constituent Comprehensive Service Plans are consistent with Housing First and Harm Reduction principles/practices
- Support and participate in constituent social and recreational activities in line with constituent individualized plan.
- Monitor documentation, records and support team to develop positive working relationships with constituents
- Ensure that all staff members work in a way that demonstrates an understanding and commitment to the rights of constituents being served
- Support staff in providing outreach, case management, counselling, advocacy, and other needed services for constituents in any environment including: the streets, shelters, prisons, hospitals, apartments, office, etc.
- Monitors ongoing interaction between staff and constituents for the purpose of assessing actions or activities initiated by the staff member
- Provide staff with feedback and constructive criticism, when required
- Collaboration with Doorways to accept constituents for both Housing First and Non
- Housing First housing supports.
- Conduct the intake and assessment process for Stepping Forward Auxiliary program.
- Maintain and supply a thorough and accurate record of all program activity including program statistics, narratives, special events, workshops.
- Assist in the preparation, organization and facilitation of staff team meetings
- Submits program reports as required (Reaching Home, WRHA) etc.
- Provide frontline professional, emotional and psychological supports to staff and/or constituent(s) and act as a key resource
- Facilitate and manage a crisis response.
- Oversee staff reporting to ensure that all constituent activities and interactions are properly documented, accurately recorded and S.O.A.P notes are kept current.
- Complete monthly file audits to ensure that constituents files are kept up to date
- Submits travel claims for approval of transportation in a timely fashion
- In collaboration with the Coordinator of Programs identify gaps in services, new initiatives, and problem solve, make recommendations and facilitate implementation of recommendations as appropriate.
- Assist in the facilitation of program evaluation where appropriate
- Adhere to the definition of and reporting procedures legislated by the Province of Manitoba and Federal government; i.e. Housing First practices, Child and Family Services PHIA etc.
- Represents the AHWC positively in the community
- Liaises Housing First organizations/agencies and other community organizations, groups, agencies, for the dissemination of information affecting changes ‘Housing First’ process and practices

**QUALIFICATIONS**
- Masters level, Bachelor’s Degree in Social Work, or a BA in Social Sciences with an emphasis on counselling services, or a combination of education and experience deemed equivalent by AHWC
