**Parkdale Queen West Community Health Centre (Parkdale Queen West CHC)** is a dynamic, community

**General Responsibilities**: Striving for excellence in client-centred care and education, you’ll work as part of the
multidisciplinary, integrated Supervised Consumption Services (SCS) staff team, within a harm reduction
framework, and in alignment with the vision and values of PQWCHC. In so doing, you’ll help ensure that SCS
operations comply with all exemption requirements and guidelines, as well as PQWCHC policies and procedures.
In the role of SCS Health Promoter, you will also work collaboratively with other interdisciplinary team members,
health care professionals and providers, and clients in identifying and resolving health and social issues, and
integrating care.

**Specific Responsibilities**: Reporting to the Manager - Urban Health/Harm Reduction, you will supervise clients
administering their own drug injections and using other permitted modes of consumption (i.e. intranasal and oral)
who may present with a variety of acute and chronic illnesses/diseases and complex health and social needs. As a
Health Promoter, you will provide education and training on harm reduction, overdose response, use of overdose
prevention tools. You will also provide safer injection and consumption, monitor clients for adverse reactions,
assess, respond to, and manage overdose events, and initiate emergency responses, as necessary in or around
the SCS. You will also develop and provide harm reduction-based health promotion initiatives and materials, support
crisis management and quality improvement efforts, lead client advisory groups and client engagement activities
advocacy to address health and well-being issues You will make referrals to other health and social service
providers as required. The Health Promoter is responsible for data gathering and the coordination of reports.
Additionally, the Health Promoter leads the development of initiatives to respond to emerging needs and trends
surrounding the SCS environment.

**Qualifications**:

- You have a graduate degree from a recognized university in a relevant discipline, or significant related

experience, combined with 3 to 5 years in health promotion within a community setting, working with
homeless and marginalized populations, including people who inject illicit drugs.
- Significant experience with harm reduction program delivery, values and strategies, and in crisis response,

de-escalation, conflict resolution and emergency response, and a background in program development,
implementation, monitoring and evaluation, and in overdose prevention and response.
- Confident in asserting and maintaining strong boundaries, you bring substantial knowledge of drug use,

specifically injection drug use, acquired through a combination of work experience, lived experience and/or
education or training, are familiar with community services and resources, and respect and value the
diversity of communities and individuals.
- Experience addressing health related system gaps through assessment, analysis, project planning,

implementation and evaluation. Experience creating and facilitating learning strategies and educational
plans for various stakeholders.
- Experience in the following areas: working in a supervised consumption or overdose prevention service;
providing overdose training, including Naloxone administration; and working with people who use drugs,

with mental health challenges, and people who have experienced trauma.
- Strong communicator with demonstrated sensitivity to others, an active listener who is open, non
- judgmental, highly flexible and resourceful, willing to learn, and able to multi-task, handle conflict and
maintain a positive outlook.
- Proficiency in one or more languages other than English would be an asset.

**Compensation**: $65,293 to $77,048 per annum plus HOOPP (Healthcare of Ontario Pension Plan) option.
