The Community Healthcaring KW provides a broad range of services aimed at improving the health of individuals and the wider community. Our services include medical and dental care, health promotion, illness prevention and outreach programming. Our expertise and resources are focused on working with those who experience health inequities and barriers to accessing health services. Collaborating as an interdisciplinary team, we provide services and supports that address the social determinants of health including factors and barriers such as poverty, inadequate housing, food insecurity, substance use, language and culture, as well as stigma and social exclusion. The Community Healthcaring KW works with the most complex client population served by primary health care professionals within Waterloo Region. Our highly skilled and diverse team works from a strength-based approach with individual clients as well as the broader community. We collaborate with many partner agencies and organizations within Waterloo Region. Our team consists of Family Physicians, Nurse Practitioners, Registered Nurses, Social Workers, Dietitians, Chiropodists, Pharmacists, Psychiatrists, Health Promoters, Community Health Workers, Administrative Staff and Volunteers.

**Job Summary**:
The primary focus of this position is to provide support to the Community Healthcaring Kitchener
- Waterloo (CHCKW) outreach teams that serve individuals who are homeless and precariously housed. Often, these individuals are more complex due to mental health and addiction issues. This position works closely with outreach clients and provides support to mobile primary care teams. In addition, our outreach teams work with other partners of the Inner City Health Alliance that serve the same clients. This role would be located at the **King Street Shelter.**
Work hours are primarily daytime but may include some evenings.
**Responsibilities**:
The outreach worker will provide support to individuals who are experiencing homelessness and insecure housing, struggle to cope with mental health and/ or use substances and support the Outreach IPC team.
**Client Services**:
- Assess client status and service needs at outreach locations.
- Provide system navigation, community connections and crisis intervention as applicable.
- Support clients with making appointments, arranging transportation, and during appointments within the community.
- Accompany clients while attending virtual appointments with psychiatric specialists.
- Work with client to develop appropriate plans of care and health goals in collaboration with the Outreach team and community partners.
- Provide mutually agreed upon services by utilizing a variety of techniques and approaches that are trauma
- informed, strength-based, culturally sensitive, and appropriate to clients’ concerns.
- Facilitate client referrals for internal and external community services.
- Coordinate and liaise with external agencies that support and collaborate with the Outreach IPC team.
- Identify barriers to health and facilitate access to resources.
- Attend and arrange case coordination meetings as requires with the Outreach IPC team.
**Health Promotion and Advocacy**
- Provide appropriate client advocacy services as directed by the client.
- Advocate on behalf of clients with Ontario Works and the Ontario Disability Support Program.
- Collaborate and consult with the Outreach IPC Team, partnering organizations and the broader community to facilitate comprehensive care.
- Participate in outreach activities.
- Engage in evidence-based practices and on-going learning.
- Advocate for individual client needs to ensure access to effective care, follow up and resources from both an individual and systemic level.
- Facilitate knowledge translation by educating colleagues on topics related to client care.
- Initiate and/or participate in systemic advocacy activities directed at achieving health equity by improving social determinants of health, eliminating stigma, and developing healthy public policy.
**Administrative Responsibilities**:
- Perform a variety of administrative tasks according to established protocols and utilizing the Heath Centre’s electronic medical records system, such as:
- Updating client information
- Scanning documents into client charts
- Scheduling appointments
- Processing referrals
- Transfer client records
- Contact external parties such as health professionals, clinics, laboratories and diagnostic facilities and hospitals to obtain client records, test results and consultation reports.
