Community Rehabilitation Worker

Cogni Care Inc. was established in 2000 and is locally owned and operated. We provide community-based rehabilitation and support services for people living with, or recovering from, mild to moderate adult-onset neurocognitive disorders. Most of our clients have experienced an acquired brain injury.
Our team consists of doctorate level researchers and educators, occupational therapists, program leaders, and rehabilitation workers. We work together to deliver services that are informed by research and best practice while striving to maintain value alignment in our work.
We are currently seeking a Community Rehabilitation Worker (CRW) to join our team in a part-time or full-time role.
As a CRW you will work one-to-one with clients in their home and community settings, delivering a participatory, person-centered model of rehabilitation and support. You will promote and maintain independence for your clients with cognitively complex daily living activities. As you gain experience and capacity, you may move from an initially supportive to a more rehabilitation focused role. There is no personal care involved with this position.
Successful applicants will receive an initial 20 to 30 hours of paid training, complemented by overlapping introductions to multiple clients with current staff. A subsequent 8 hours a month of paid training will be provided for the first 4 to 6 months of employment.
Examples of the services you may provide include:
- Developing and supporting independence with organization, goal planning, managing paperwork, and scheduling and attending various appointments.
- Identifying points of community integration and facilitating involvement.
- Assisting clinicians to achieve client rehabilitation goals.
- Maintaining safe and stable housing.
- Building and maintaining healthy routines with eating, socializing and exercise.
This position requires excellent written and verbal communication skills, problem solving abilities, and the capacity to effectively work independently and as part of a team. All members of our organization do some work from home offices, using custom online programs for communication and collaboration. A willingness to learn, and to adopt, these programs is required. Staff with a regular schedule are also provided with a late model cell phone and a robust data plan for use during their working hours.
CRW scheduling offers flexibility to complement an academic schedule, family responsibilities, or other commitments. The starting rate for this employment position is $26.11/ hour (plus vacation pay); reimbursements for client transport expenses are provided in addition to the hourly rate.
**Job requirements**:
- 2-year diploma and/or education in social services or a related field (preference may be given to applicants with a 4-year degree or continuing education plan).
- Minimum 1 year of experience supporting adults with cognitive impairment and/or disability (preference may be given to those with additional experience).
- Recent clear vulnerable sector criminal record check (i.e., CRRP criminal record check; our agency will cover the cost).
- A reliable vehicle suitable for visiting and transporting clients in the community, with ‘use for business’ insurance.
- BC driver’s license and a driving abstract demonstrating safe and responsible vehicle operation.
- Valid first aid certificate (CPR and wound care).
- Competency using, or willingness to learn, Microsoft Office 365 and other online platforms.
- A secure personal computer and Internet connection (or willingness to use provided data plan and cell phone for internet access with personal computer).
**Experience in, or a willingness to learn about, the following areas is also an asset**:
- Supporting people to develop person-centered goal plans based on their current abilities and preferences.
- Developing adaptive strategies to support independence.
- Understanding relationships between brain health and functional independence.
- Understanding how executive function, attention and memory inform independence with complex living skills.
- Identifying community resources and effectively facilitating access and integration.
- Understanding the cycle of emotional crisis and how to effectively intervene.
