Toronto General Hospital (TGH) is leading the way in cardiac care, organ transplants and the treatment of complex patient needs. Home to three of UHN's major program groupings (Heart and Circulation, Transplantation and Advanced Medicine and Surgery), our highly skilled teams and dedicated volunteers are committed to UHN's vision of achieving global impact.

The Immunodeficiency Clinic provides specialized outpatient medical monitoring and treatment for HIV/AIDS disease and its complications. We also provide counselling to help patients deal with related pharmacological, psychological, social, functional and nutritional concerns.

In the provision of patient and non-patient care activities, the primary responsibilities of the **SOCIAL WORKER** include: assessing patients for the purpose of evaluating the need and appropriateness for service, the nature of the problem, and formulating recommendations or treatment plans; implementing an intervention plan with patients, family members, significant others, and the multi-disciplinary team for the purpose of facilitating changes in behavior, attitudes, feelings and/or the environment; e.g.(discharge planning adjustment to illness); reassessing and modifying the intervention plan as required; establishing and maintaining liaison with appropriate community agencies; establishing contact with members of the multi-disciplinary team and community agencies to obtain, provide or exchange information relating to patients' psycho-social functioning and health care in the past, present and future; rendering services for the immediate benefit of the Hospital and the community, performing activities required for the effective and efficient operation/maintenance of the Immunodeficiency Clinic and Department of Allied Health (Social Work); performing other duties consistent with the job classification, as required; provision of clinical teaching to undergraduate (B.S.W.) and Graduate (M.S.W.) Social Work Students, Medical Students and Professional Colleagues at the University Health Network; participation in presentations at professional conferences, conduct and collaborate in relevant research activities and projects and publish associated material.

**Qualifications**:

- Completion of a Masters Degree in Social Work (M.S.W) or recognized equivalent
- Eligibility for membership with the Ontario College of Social Workers & Social Service Workers (O.C.S.W.S.S.W.)
- Specific program may require specific additional training e.g., sexual health and reproduction, statistical data management
- Experience working with patients and families living with HIV an asset
- Experience working with a multi-disciplinary team in a health care setting
- Research experience/education in HIV-related domain an asset
- Student education/supervision an asset
- Excellent verbal and written communications skills
- Excellent interpersonal, time management and organizational skills
- Excellent problem solving skills
- Knowledge of policies, legislation, program's and issues related to social welfare institutions and services
- Knowledge of a second language an asset
