**Position Description**:
**Here’s What You’ll Get to Do**:
The Connected Care - Patient Care Information Coordinator will collaborate in coordination of in-person and virtual services delivered from hospital to home and community care. The CC-PCIC will be part of a small, innovative and high-functioning team, enjoying flexibility in how work is completed while meeting priorities and emerging demands of the clinical program.

In the role, the Connected Care - Patient Care Information Coordinator will work with others and support diverse care coordination and program administrative functions including:

- Communicate with inter-professional team members within and outside of the organization to coordinate the booking of virtual visits and clinic schedules
- Ensure scheduling of family and homecare provider education and related services is processed, organized, updated and communicated effectively
- Support the registration and flow of patients on Connected Care pathways and within the clinic (virtual and in-person)
- Support team in organizing and storing diverse materials (teaching supplies, emergency kits) used in Connected Care service delivery
- Collate relevant information about clinical and learning needs and relay these in a precise and timely manner to colleagues within and outside of the organization
- Greet and orient patients, family caregivers, home and community providers and vendors to Connected Care and SickKids
- Assist in the coordination of family/patient and health care professional education sessions and events
- Provide administrative support including calendar management, mailing, faxing, scanning, photocopying, and maintaining electronic filing systems
- Prepare original documents, including letters, reports, spreadsheets, presentations, and educational material
- Organize and coordinate staff schedules and updating systems as required and communicate employee scheduling/staffing issues to management and relevant team members
- Act as the primary payroll administrator and provide support to leadership team and staff related to payroll issues.
- Maintain confidentiality of all patient and staff personal and health information

**Here's What You'll Need**
- Bachelor’s Degree in a related field (Medical Administration, Business Administration, Health Administration) preferred
- Minimum of 3 years clerical or administrative experience in a health care setting preferred
- Experience in a hospital environment is preferred
- Scheduling experience and medical terminology is an asset
- Accurate keyboarding skills and strong working knowledge of Word, Excel, PowerPoint, and Outlook
- Experience with EPIC and scheduling platforms is an asset
- Excellent problem solving skills that demonstrate an ability to anticipate potential issues and make suggestions
- Strong interpersonal, communication and organizational skills with both fellow colleagues and patients
- Ability to remain focused and prioritize tasks in a busy and ever-changing environment
