DUTIES AND RESPONSIBILITIES:
The Patient Care Manager (PCM), Healthy Aging is responsible for the management of the clinical service area and providing visible leadership to the interdisciplinary team to ensure optimal performance management and smooth operation of the service area and high quality patient care. As a transformational leader, the PCM will create a vision to guide positive change and innovation through inspiration, motivation, role modelling and coaching and by working in collaboration with the leadership and interprofessional teams in the Healthy Aging Program. The PCM facilitates and supports implementation of evidence based practice to achieve excellence in high quality delivery of patient care.

The Patient Care Manager, Healthy Aging also works closely with the Program Director, Patient Care Managers and Clinical Practice Leaders, to lead and guide specific key quality improvement, strategic and performance improvement initiatives identified as priorities in the Healthy Aging Program. Integrating strategy, quality improvement, practice and project planning across all services within the Healthy Aging program to achieve deliverables and excellence in patient care is a key component of this role. The PCM also contributes to the implementation of continuous improvement in the program to achieve excellence in person-centered care through the utilization of skills and attributes associated with the fields of process improvement, project management and change management in an integrated manner.

QUALIFICATIONS:

- Bachelor degree in a health related discipline;
- Master's degree or current enrollment in a Master's program (preferred);
- Current membership in good standing with a regulated professional College in Ontario;
- Minimum of two (2) years recent clinical/leadership experience;
- Minimum of two (2) years' experience in quality improvement development, implementation and evaluation;
- Educational preparation in quality improvement, certificate in quality improvement or equivalent in the use of quality improvement methods and tools;
- Project Management skills and experience
- Demonstrated knowledge in related clinical specialty;
- Knowledgeable in the principles of adult education and teaching/learning principles.
- Knowledge of current trends in nursing, allied health and health care as well as applicable legislative and hospital/program policies;
- Demonstrated knowledge of quality improvement principles, methods, and tools with excellent project management skills including assessment, planning, implementation and evaluation skills at a program and corporate level
- Demonstrates excellent verbal and written communication.
- Strong analytical skills including the ability to collect, analyze, share and interpret qualitative and quantitative data
