Clinical Leader

This leadership role will provide strong flexible and resilient support to the staff in support of the organizations goals and objectives. The Clinical leader - Transitional care and CCC will:

- Collaborate with Patients, families and the health care team to provide professional practice that respects the rights of patients
- Provide support and coordination to both the Complex continuing care and the transitional support programs, including Hospital to home and the transitional care beds.
- Work closely with the interdisciplinary team to ensure appropriate usage of the designated beds. The coordinator will be responsible for reviewing referrals, sending offers, attending interdisciplinary rounds to ensure goals are being met, and ensuring barriers are reduced for patients to move through the program.
- Integrate a patient-centred approach to care that is focused on enhancing and coordinating the care for patients living within, or part of the Muskoka Algonquin Ontario Health Team catchment area.
- Create seamless care coordination for patients across all sectors of the healthcare system.
- Assess and prioritize each potential referral -directing to the lead agency for those that qualify while redirecting to other resources (such as the PFN or HCCSS) for those who do not.
- For Hospital to Home patients
- work closely with the Agency lead coordinator to ensure that each referred patient has a coordinated care plan and ensure timely but seamless transition home.
- For Transitional care patients
- Ensure a well-coordinated transition into the transitional care beds including ongoing coordination of discharge plans into the community.

**REQUIRED EDUCATION**

Registered with C.N.O.

Current certification in BLS, WHMIS, Fire Safety and Non-Violent Crisis Intervention/Mandt

**REQUIRED EXPERIENCE**

Minimum 3 years of clinical experience in a hospital setting

Two years related experience providing program leadership in a healthcare setting

Demonstrated skills in managing complex inter-person relationships in a confidential, caring and compassionate manner

Functions inter-professionally, collaboratively and effectively across nursing units

Ability to demonstrate multi-tasking

Strong computer skills and familiarity with Cerner

**DESIRED EXPERIENCE**

Previous leadership experience in a Team Leader or formal Clinical Leadership role

Leadership Certificate

May be assigned to accompany patients on transfers

Willing to work 7.5 hours shifts - Monday through Friday

As a condition of employment, external hires are required to submit proof of COVID-19 vaccination to the Hospital’s Occupational Health and Safety department.

**Salary**: $36.24-$51.02 per hour
