**BASIC PURPOSE**

As the **Age-friendly Communities Program Lead** **(AFC)** for our AFC program, you play a key role in providing Age-friendly Communities planning knowledge and expertise to the organization and community partners across the province. In this position you work closely with the Executive Director to ensure project deliverables are linked with program strategies and you provide leadership and guidance for the design and development of key program offerings such as curriculum, webinars, resources, workshops, and planning processes. You work collaboratively with BCHC staff to plan, develop, and deliver various healthy communities’ offerings, and you provide planning and community-wellbeing expertise to the partner organizations and communities we serve.

As part of our team, your role will be to specifically coordinate and deliver our provincial Age-friendly Communities Program. You will be working in a highly collaborative learning environment and will take that learning out to communities to build on their capacity for developing healthier communities for healthy aging. Through your work you will be working closely with multi-sectoral groups that include municipal governments and regional districts, regional health authorities, First Nations and not-for profit and community organizations, to build capacity for collaboration and action.

- Work closely with the Executive Director to ensure alignment of program strategies and implementation.
- Coordinate and facilitate program planning, design and work planning meetings with staff and external partners.
- Provide oversight and guidance to program staff on the design and development of program offerings such as webinars, workshops, curriculum, planning processes.
- Work closely with communication, grant coordinator and evaluation staff to ensure strategies are meeting program goals and objectives.
- Provide planning and content guidance and work collaboratively with program staff around grant administration, research, and delivery; and
- Work with external multi-sectoral groups which may include Indigenous and non-Indigenous local governments, regional health authorities, not-for profit and community organizations as potential partnership opportunities arise.

**RESPONSIBIILTIES**

Coordination/Project Management
- Day to day coordination, oversight/guidance of program deliverables.
- Provide ongoing project management supports for program deliverables (updating project management software, support development of funder and other reports).
- Coordinate/facilitate planning and design meetings (with staff and externals).
- Outreach and engage with communities and partner organizations.
- Maintain contact lists, inventories, etc.
- Work with communications team to provide input into a range of communications (blog posts, stories, etc.) to help promote the program.
- Work with evaluation staff to ensure goals and objectives guide design and implementation.

Design/Development/Delivery
- Provide leadership and expertise to staff around planning, design and delivery of our offerings.
- Design and deliver capacity support and processes for communities that are suited to their unique contexts, and that support the creation of community age friendly health and well-being policy and/or programming across BC (through activities such as: research, analysis, community engagement, policy analysis, needs assessments, engagement strategy design, recommendations reports, etc.).
- Design and facilitate workshops, webinars, and other BCHC capacity building events and activities.
- Pull together multi-sectoral partners and fostering collaborations and partnerships to support age friendly healthy community initiatives.
- Support BCHC staff and our partners (local government, health authorities, Indigenous communities) to navigate community health and well-being/public health related contexts.

Monitoring and Evaluation
- Contribute your knowledge of public health and the social determinants of health and methodologies to ensure resources are relevant to health authorities, indigenous and non-indigenous governments, and community health promotion focused audiences across a wide range of focus areas.
- Provide strategic planning support and ensure program delivery activities are aligned with organizational and program goals and objectives.
- Provide a leadership role with key BCHC program team members to plan and coordinate logistics for workshops, events, planning processes and the development of tools and resources for community partners.
