**Job Overview**
Humber Meadows Long Term Care is seeking compassionate and dedicated Restorative Care Aides to join their team. Reporting to the Director of Care, the RCA is responsible for providing restorative care to residents to achieve maximum independence eg: feeding, mobility, personal care, dressing/grooming, transferring, and toileting.

Under the direction of Director of Care, the Restorative Care Aide (RCA) provides assessment, restorative program assistance, care, monitoring, and documentation to identified residents so that they may maintain and improve their personal independence. The goal is that each identified resident will work towards achieving a maximum level of independence, which may include restorative feeding, mobility, personal care, dressing/grooming, transferring, and toileting.

**Duties and Responsibilities**:
**Restorative Care**:

- Provides restorative care program to designate residents under the direction of the Director of Care. The program includes feeding, mobility, personal care, dressing/grooming, transferring, and toileting.
- Participates in the Continuous Quality Improvement (CQI) Program.
- Prompts and coaches the resident using specialized equipment and adaptive prosthetic devices under the direction of the restorative team program
- Identifies and builds on the resident’s level of functioning, such as capacity building like emotional capacity, awareness of the environment, social capacities, ability to communicate and ability to make logical connections.
- Monitors, documents, and reports on the resident’s self-developed goals.
- Documents appropriately on resident multi-disciplinary progress notes including assessments.
- Participates actively in restorative care and interdisciplinary teams and consults appropriately with care team members.
- Demonstrates an understanding of the cognitive/social/emotional requirements of the residents by adopting appropriate resident-specific approaches and interventions.
- Demonstrates safety consciousness by practicing good body mechanics and lifting techniques. Recognizes safety hazards and takes appropriate actions.
- Takes initiative to promote safety. Supports resident safety programs and policies. This includes reporting unusual occurrences including near-misses, participation in resident safety initiatives and problem solving, and identification and prevention of key indicators
- Complies with the Occupational Health and Safety Insurance Act and WHMIS information system. This includes recognizing health and safety hazards, reporting incidents, fulfilling responsibilities under the applicable legislation, as well as participating in in-service and fire drills.
- Other duties as may be assigned by the Executive Director

- Ensures documentation of the restorative care provided and other findings, in a timely manner in accordance with HMLTC policy and regulations;
- Self-actualization by helping residents to reach their maximum potential;
- Strives to provide excellent resident centered care**;**:

- Build professional relationships with residents, families, co-workers and the interdisciplinary team; become a trusted member of the residents circle of care;
- Adheres to all HMLTC Policies and Procedures, Infection Control protocols, LTCHA and Regulations, and Occupational Health and Safety policies
- Other duties as assigned

Expected hours: 40 per week

- Long Term Care: 1 year (required)
- Restorative Care Aide: 1 year (required)
