- **40hrs per week, Monday-Friday 8:30am-4:30pm, 4 weeks’ vacation.

As one of the largest provider of Housing Focused Shelter in the Niagara Region, the YWCA Niagara Region offers innovative housing programs and services that provide hope for women and their families living in poverty.

The YW will be operating Oakdale Commons, with 15 Home For Good units. The Home for Good Program aims to increase successful housing tenure to individuals experiencing chronic homelessness by providing community support to individuals transitioning from the shelter system, street living, hospitals and/or criminal justice institutions to permanent housing.

To complement our Housing Focused Shelter program staffing component, the YW is looking for an exceptional team player, who is compassionate, non-judgmental, and self-motivated to round out the team.

Position SummaryThis is a mid-level position within the YWCA’s Housing Program. The Home For Good Worker’s primary role is to work with the Home For Good clients to provide individualized and flexible support while supporting the clients in maintaining their housing. The Home for Good Worker is to ensure that the individuals with the most complex needs are able to live and thrive in their community. The Home For Good Worker is to meet with the clients at Oakdale Commons to assist in identifying long term goals and short term objectives in relation to maintaining housing. In addition, the Home For Good worker participates in individual and systemic advocacy to improve resource access and availability.

The Full Time Home For Good Coordinator will report to the Manager of Transitional Housing to assist in the operation of Oakdale Commons in accordance with agency policies and procedures. Which includes the following duties; attending regional Home For Good meetings and trainings, maintaining appropriate record keeping, complete move in/move out forms, maintaining health and safety for residents/staff/facilities, long term goal setting and short term objective with referrals to other resources, promote co-operative and independent living, participate in housekeeping duties including promotion and role models of life skills.

**Responsibilities**
- Ensure deadlines are met and administrative procedures are followed
- Responsible for inputting all information into HIFIS system regarding work done i.e. intake, discharge, case planning, etc.
- Collection of housing charges, rent calculations and managing arrears
- Record monthly and fiscal year statistics and impact stories as needed by Executive Director
- Maintains the highest level of confidentiality and boundaries regarding client information and the sanctuary and safety of Oakdale Commons residents, staff and community partners
- Maintain an individual caseload of 10, supporting through case planning, goal setting and referrals. Meeting with clients minimum once a week as per regional standards. Ensure necessary referrals to assist with goals achievement.
- Participate in Move in/Move Out plans and procedures.
- Assisting the residents in the development of personalized safety/crisis plans. Coordinating individual care plans and sit on round table care meetings as necessary.
- Support the residents to follow rules and responsibilities of the building and guidelines of the program
- Performing a visual health and safety unit walkthroughs as per regional policies and procedures
- Supporting participants utilizing harm reduction principles and programming
- Report to designated Program Manager periodically on team and individual work accomplishments, problems, progress in mastering tasks and work processes, commit to regular one-to-one check in appointments with Program Manager to discuss caseload - concerns, successes and guidance needed once per month
- Develop and maintain ongoing working relationships with Community Partners within the Region
- Ensure a safe and hazard free environment for staff, sub-contractors and clients as per the YWCA Niagara Region’s Health and Safety Policies by notifying Director of Operations of any Health and Safety concerns in a timely manner.
- Will be required to be part of the on call rotation schedule, training in shelters required for this duty

**Qualifications**:

- Minimum undergraduate degree/diploma in Social Work/Social Science or related field
- Minimum 2+ years working with the chronically unhoused population.
- NVCI or CPI, ASIST, Mental Health First Aid, and Naloxone training considered an asset.
- First Aid, CPR (Level C) mandatory.
- Basic computer skills and a willingness to participate in on-going training
- Vulnerable Screening Clearance (Police Check)
- General knowledge of Landlord and Tenant Rights and Residential Tenancies Act, Ontario Mental Health Act.
