**DUTIES**:
The **Transitional Housing Support Worker**, under the direction of the Executive Director, will provide physical, mental, emotional, and spiritual case management and support to Indigenous women and children seeking stable housing.

**RESPONSIBILITIES**:

- Provide housing supports, resources, and services to NWC program participants and be a community support for referrals from other agencies supporting Indigenous women and children.
- Engaging with clients and community members in a respectful, non-judgmental and supportive way that helps to improve their health and well-being, develop life skills, increase their sense of belonging, and build resilience;
- Incorporate Indigenous Culture as a way of being and knowing in the organization;
- Complete intakes, safety plans, case notes, incident reports, plans of care, etc. within transitional housing environment.
- Participating in case conference(s) / review(s) and all relevant case management meetings.
- Reporting all disclosures, safety plans and serious occurrences to appropriate contact.

**QUALIFICATIONS**:

- College diploma in social services or closely related field and a minimum of 3 years’ experience in associated field;
- Knowledge of the homeless and housing sectors in the Greater Hamilton Area, including resources;
- Experience working in an Indigenous organization is an asset;
- Demonstrated experience in case management supporting people with diverse needs;
- Ability to advocate on behalf of clients to facilitate access to identified supports;
- Intermediate computer literacy skills including Microsoft office suite and databases;
- Demonstrates cultural competency including awareness, knowledge and understanding of the impacts of colonialism;
- Non-violent Crisis Intervention and Trauma Informed Training;
- Must be available to work some evenings and occasional weekends;
- Must possess a valid driver’s license with a clean driving record;
- Must have or obtain, within 30 days of hiring date, a Vulnerable Sector CPIC.

**SALARY**: $23.00/hr

**JOB TYPE**:Full-time 4pm-12am Monday - Friday
