We are looking for a sensational LPN or RN nurse to join our leadership team on Vancouver's North Shore.

As a Client Care Coordinator, your primary responsibilities will be:
- Meet with potential clients and family members to discuss their needs and provide solutions in the form of a service plan. It's a great way to support seniors in our community
- Work with Home Care Consultant to meet with existing and potential referral partners to share valuable community resources, information, and to grow our business through referral partnerships.
- Give health related talks and information sessions to our community partners.Training and support for this provided in full, and we are great at developing this skillset.
- Work with the Home Care Consultant to Conduct Service Inquiries and Care Consultations as needed.
- Conduct client/Care Professional introductions with every new client and with every new Care Professional.
- Work with other team members to coordinate various aspects of a client's care
- Performing quality assurance visits and documenting all meetings
- Recognize and pursue opportunities to modify service plans to be support ongoing needs of clients through quality assurance program.
- Demonstrate open and effective communication with clients, family members, and other team members
- Work with clients and their families on issues that may arise in order to find and execute a resolution.**And last but not least**: Building trust, taking the lead, sharing your heart and being a key player in making a meaningful and practical difference in client's lives and wellbeing.
**Education/Experience Requirements**:
- College degree preferred (RN or LPN preferred but not necessary)
- One year experience in home care, health care or senior-related industry required, an equivalent combination of education and work experience may be considered.
- Must possess a valid driver's license
