The Transplant Clinics at St. Joseph’s Healthcare Hamilton are responsible for coordinating over 130 deceased and living donor kidney transplants annually and caring for more than 1300 post-transplant patients. Primarily located at the Charlton Campus, with a satellite clinic at Niagara Health, the Transplant clinics involve many members of the interdisciplinary health care team to ensure safe, effective, and timely care across the continuum in collaboration with community partners, referring hospitals, volunteers, and inpatient unit. The multi-site Nephrology Clinics at SJHH are comprised of a variety of clinics, including General Nephrology, Hypertension, Transitional Care, Genetics, Glomerulonephritis, and Multi-care Kidney Clinics.

Reporting to the Clinical Director of the Kidney Urinary Program, the clinical manager is a member of the leadership team that is responsible for the coordination, direction and program development of the Transplant and Nephrology Clinics as well as the human and fiscal resources. The CM collaborates with the Medical Director, Clinical Director, Educator, and other interdisciplinary healthcare team members to ensure that effective, efficient quality patient care services are delivered as informed by evidence-based best practice guidelines. The CM is an active participant and leader in the development and implementation of provincial Ontario Health initiatives.

The CM is accountable for planning, implementing, and integrating plans that are in alignment with the department, and with the mission, vision, and philosophy of the organization. They are responsible to ensure that the staff within the clinical area operate within their professional scope of practice as defined by their regulatory body. The CM is responsible to ensure that all staff meet and provide quality, safe patient care and additionally plays a key role in providing opportunities for the professional development of all staff.

**QUALIFICATIONS**:

- Undergraduate degree, required
- Master's degree, preferred
- Registration with appropriate regulatory college
- Canadian Association of Nephrology Nurses and Technologists (CANNT) professional membership an asset
- 1 to 3 years recent and successful clinical and management experience, preferably in Transplant, Outpatient Clinics or Nephrology
- Demonstrated knowledge of relevant area of clinical practice
- Demonstrated evidence of highly effective critical thinking, problem solving, innovative solutions, and challenging accomplishments
- Excellent organizational and interpersonal skills
- Demonstrated ability to facilitate the successful achievement of outcomes
- Excellent team building skills
- Experience managing fiscal resources within the allocated budget
- Advanced knowledge of management principals

**RESPONSIBILITIES**:

- Hires, promotes, performance manages and terminates staff, as required
- Manages employee relations issues, promoting a teamwork environment where staff can interact productively and efficiently
- Develops, implements and communicates performance measures; conducts regular performance appraisals of direct reports, providing ongoing coaching and feedback
- Articulates the performance expectations of all interdisciplinary staff assigned to the units
- Fosters effective employee and labour relations through knowledgeable interpretation and implementation of hospital policies and collective agreements
- Develops and implements appropriate orientation programs for new staff in collaboration with educator
- Assigns, supervises and leads appropriate activities of all staff and volunteers
- Leads staff in problem solving and decision-making by ensuring administrative policies and procedures are followed
- Develops, implements, and manages short and long-term strategic priorities
- Manages the development, implementation, evaluation and maintenance of quality improvement initiatives which are in alignment with established SJHH standards
- Monitors quality improvement outcomes on a regular basis; develops action plans to address identified issues
- Coaches staff to ensure that continuous quality improvement initiatives are incorporated into day-to-day activities
- Ensures excellence in patient care delivery in accordance with MOHLTC, Health Canada and CCHSA standards and Ontario Health (Ontario Renal Network, and Trillium Gift of Life
