Cassellholme is seeking a **RAI-CHA ASSISTANT **to work in our Community Support Services Program as a inter-professional health care team member.

Under the direction of the Manager of Community Support Services (CSS), the RAI-CHA Assistant is responsible for administering the Resident Assessment Instruments-Community Health Assessment (RAI-CHA) and collaborating in case management, service arrangement and coordination.

**What can I expect to do in this role?**

The RAI-CHA Assistant will undertake the following key responsibilities as they pertain to the community agencies that provide access to clients who will be assessed by utilizing the RAI-CHA:

- Develop a schedule of assessments;
- Conduct assessments and report regularly to the community partner agency;
- Collaborate in case management, service arrangement and coordination;
- Communicate on a regular basis with other community agencies, including the CCAC, hospitals, physicians, and pharmacies;
- Complete referrals to other CSS agencies, as required
- Ensure the appropriate agencies receive complete documentation
- Ensure that Health Teaching is completed as necessary during the assessments and documented
- Input data into AlayCare;
- Upload RAI-CHA assessments to the IAR;
- Ensure that clients are advised of their rights and the complaint process established by the community agency’s protocol;
**How do I qualify? Mandatory Requirements**:

- Graduate of an approved Registered Practical Nurses Diploma program
- Current certificate of competency with the College of Nurses of Ontario
- Three (3) years of nursing experience, preferably in community
- Proficient computer skills (AlayaCare, Word, Excel), knowledge and demonstrated skills and experience with RAI-CHA
- Driver’s license and own vehicle required for travel throughout the District of Nipissing;
- Excellent time management skills and ability to work independently with clear aptitude for detail oriented work.
- Experience with assessment, interviewing and health teaching with adults
- Comprehensive knowledge of aging and community programs
- You have a satisfactory CPIC with vulnerable sector screening;
