**About the Organization**:
The Barrie Area Native Advisory Circle (BANAC) is a regional social planning and development organization which provides innovative programs and service to Indigenous communities in Simcoe County, Muskoka, and the northern York Region, including wholistic Health Services through the Mamaway Wiidokdaadwin Primary Care Team. As an Interprofessional Health Care Team, Mamaway team members provide primary care, mental health, addiction services, traditional healing and health promotion services for Indigenous communities, grounded in Indigenous wellness practice. We provide wholistic services in clinic locations, community and in partnership with mainstream service partners.

**About the Role**:
As a part of the interprofessional primary care team, the Case Manager’s role is to support the development of a comprehensive supportive treatment plan, coordinate physical, emotional, mental, spiritual, and social health services, and to make appropriate referral and linkages to community services and resources. The Case Manager provides services in a supportive, confidential, and culturally safer manner to help clients work towards their goals and access the resources they need to fully participate in their health care services.

The Case Manager is responsible for case management specific screening, assessing, planning, connecting, and transitioning of care. The care provided by the Case Manager is time-limited and goal focused. The Case Manager will participate in rounds, team meetings, care conferences, and client appointments as necessary to support the fulfilment of their case management plan. This role will work collaboratively with therapists and social workers but the Case Manager does not provide therapy or counselling to the clients they support.

As an important part of the Red Road to Recovery team, the Case Manager will complete case management and administrative coordination duties as assigned by the Program Coordinator or delegate.

**What You Bring**:

- Completion of a Social Service Worker Program or a Diploma or Certificate in Mental Health and Addictions or an equivalent combination of education and experience.
- 3 years minimum of Case Management experience with experience supporting clients with mental health and addictions needs. Completion of training in case management considered an asset
- Demonstrated experience working with Indigenous communities and clients; ability to provide culturally safe care, including knowledge of Indigenous community and culture
- Trauma-informed practice skills applied to supporting clients with complex social determinants of health needs
- Experience providing case management services to clients with complex needs in the social determinants of health, particularly homelessness, mental health and addictions, and transportation.
- Experience in implementing risk management and organizational protocols
- Demonstrated knowledge of health, social and community-based resources required to provide case management services
- Case management assessment skills to develop an understanding of client needs in physical, psychosocial, emotional, spiritual and cultural domains
- Team work with interprofessional team members internal to an organization and external to an organization
- Relationship building skills with clients, social networks, and community service providers

**Information About Working at BANAC Mamaway Wiidokdaadwin**:

- Work is typically performed in clinic, office and community settings.
- Regular travel to meetings and communities, will be required - vehicle an asset
- Regular overtime work may be required from time to time, BANAC team members enjoy a flex schedule agreement
- Periodic exposure to smoke may occur from the burning of sacred medicines: Tobacco, sweet grass, sage or cedar.

Specific employment conditions are specified in an employment contract with BANAC and include the following:

- Valid C.P.R. and A.E.D. Certificates required
- Valid Class G Ontario Driver’s License is required with clear check for vulnerable sector
