**Positions Description**:
Reporting to the Manager of the Lung Health Program, the Smoking Cessation Coach (SCC) is responsible for helping clients adopt healthy behaviors. The incumbent is responsible for implementing the clinical aspects of the Smoking Treatment for Ontario Patients (STOP) program in conjunction with the Ottawa Model for Smoking Cessation (OMSC). This position will provide psychosocial assessment, counselling, education, and behavioral intervention to clients identified as tobacco users. The SCC assists the client using a harm reduction approach to smoking cessation utilizing both counselling and nicotine replacement therapy to progress towards their smoking cessation goals. Counselling may be provided one on one or in a group setting. The SCC is responsible for collecting and entering program-related data into an electronic database as well as in the Electronic Medical Records (EMR).

***Requirements** **Education/Professional Experience**:

- Bachelor’s Degree or Diploma in social sciences or equivalent combination of training and experience
- Knowledge about community-based health care settings
- Knowledge about community mental health
- Mental health and addictions experience, preferred
- TEACH Certification, preferred
- Knowledge and understanding about working with marginalized populations
- Crisis intervention and counselling experience, preferred

**Key Competencies**:

- Familiarity with the Ottawa Model for Smoking Cessation
- Familiarity with the STOP Program
- Knowledge of acts or legislation that govern practice: Personal Health Information Protection Act (PHIPA), Workplace Hazardous Material Information System (WHMIS), Infection, Prevention and Control Measures (IPAC)
- Knowledge of primary health care and the social determinants of health
- Highly collaborative and works well with other healthcare professionals
- Knowledge of behaviour change theory with regard to tobacco an asset
- Must be able to work on multiple tasks simultaneously to meet deadlines
- Ability to work independently with mínimal supervision
- Excellent communication/interpersonal skills
- Strong organizational skills
- Successful criminal record check
- Facilitates smoking cessation groups
- Engages in ongoing learning and skills development

**Language Capability**:

- Bilingual French and English, preferred
- Other additional languages an asset
