**Community Lead, Toronto**
**Full-Time Permanent - Work From Home Position**
**Role Description**
Reporting to the Senior Manager, Ontario Community Services, the Community Lead will work with stakeholders across Toronto to provide support services for people living with ALS and their families, helping them navigate the health care system. You will also be responsible for creating and supporting opportunities for awareness, advocacy and fund development in your local region.
**What You Will Do**:
- **Support Services**_
- Manage a client caseload of approximately 100-120 clients by establishing regular communication with people living with ALS and their families to provide support and help them navigate the healthcare system
- Identify issues, manage information as well as provide non-clinical support to clients and families
- Work with Occupational Therapists, families, people living with ALS and the Equipment Program team to support the Equipment Program process as needed
- Facilitate support and/or bereavement groups and/or make referrals to community support services/bereavement services as needed
- Liaise with health care and social services professionals to support client care and advocate on their behalf as needed
- **Advocacy and Awareness**_
- Provide ALS education to healthcare providers, community agencies, service clubs, students, families, and the public as requested. Organize educational opportunities with relevant partner agencies e.g. hospice
- Organize and participate in local and provincial advocacy efforts by speaking at events
- Develop partnerships and relationships with local health care and social services
- Act as a representative of ALS within region, including community events, media interviews, speaking engagements and presentations
- Attend government meetings in relation to provincial advocacy efforts
- Fund Development_
- Lead local Walk to End ALS events, working with volunteers to implement walks within the region, drive recruitment and manage all planning, sponsorship, relationships, promotion, and media during walk preparations
- Work closely with the Events team to identify potential donors and relationships that will enhance regional fund development activities
- Participate in and/or develop Fund Development community events in the region as required, attend events as an ambassador of ALS Canada as required
- Work independently from a home office
**What we are looking for**:
- Successful completion of an accredited health discipline program and eligible for registration with professional regulatory college required (e.g., Social Work or relevant field)
- Minimum five years’ experience in a healthcare-related field required
- Extensive knowledge of the Ontario health care system and regional resources
- Excellent computer skills (Word, Excel, PowerPoint)
- Experience with CRM systems required as Apricot 360, Raiser’s Edge
- Previous experience working with volunteers preferred
- Proven fundraising track record preferred
- Strong interpersonal, communication, active listening skills
- Demonstrated excellence in relationship building, working as part of a team, problem solving, advocacy skills and time management
- Ability to stay neutral and work effectively under pressure
- Comfortable connecting with clients, teammates on a virtual platform
- Proficient presentation skills, comfortable speaking in front of a group of people
- Experience with palliative care is an asset
