Service Coordination des services (SCS) est un organisme bilingue à but non lucratif, géré par un conseil d’administratif composé de bénévoles, qui travaille avec et pour des personnes ayant une déficience intellectuelle et leurs familles dans la région d’Ottawa. SCS offre des services aux adultes et aux enfants en leur fournissant des renseignements, des services d’aiguillage et un soutien de gestion de cas. L’un des buts importants de l’organisme consiste à habiliter les personnes/familles à faire des choix éclairés concernant les soutiens qu’ils recherchent.

Relevant directement du superviseur des services aux enfants, le gestionnaire de cas est responsable de la coordination des services aux enfants admissibles ayant une déficience intellectuelle et/ou d’un trouble du spectre autistique avec la participation de la personne/famille. À l’aide du plan de service, le gestionnaire de cas détermine et aide à obtenir l’accès aux services et soutiens en remplissant au nom de l’enfant des demandes d’aiguillage aux ressources communautaires demandées, selon les protocoles de l’organisme, s’il y a lieu. Le gestionnaire de cas informe les personnes/familles et leur permet de devenir aussi autonomes que possible, selon leurs capacités d’exécuter des tâches, tel qu’indiqué dans leur propre plan de service ou celui de leur enfant.

**Responsabilités**
- Amorcer et compléter la fonction d’accueil, ainsi que toutes les exigences se rapportant aux documents applicables.
- Si une participation continue est requise, élaborer, avec la participation de la personne/famille/réseau de soutien, un Plan de service axé sur un but, afin d’aider la famille à accéder aux services et/ou aux soutiens disponibles dans la communauté.
- Faciliter la planification de la transition des 14 à 18 ans, en collaboration avec la famille et l’école que fréquente l’enfant.
- Aider les familles en leur fournissant des renseignements et un soutien d’aiguillage, afin qu’elles puissent accéder aux services et soutiens appropriés pour leur enfant dans leur communauté.
- Coordonner des conférences de cas, ou y assister, avec la famille et/ou d’autres organismes afin de donner à l’enfant/la famille le soutien nécessaire.
- Maintenir l’exactitude et l’intégrité de l’information que renferment les dossiers de cas et la base de données de l’organisme, conformément aux normes établies ; au besoin, fournir des renseignements/données et des analyses statistiques.
- Aider à l’élaboration du plan de réintégration de l’enfant identifié par la SAE et le SESSC en partenariat avec toutes les parties, suivant le protocole existant.
- Assister aux études de cas mensuelles avec le superviseur de l’équipe des enfants pour le soutien clinique, et recevoir des directives au besoin.
- Évaluer la situation et aviser le superviseur l’équipe des enfants quand les services de gestion de cas ne sont plus requis.
- Participer aux initiatives du système qui concernent les services aux enfants.
- Promouvoir SCS en participant aux présentations de l’organisme aux groupes de parents, aux écoles et autres organismes, selon les besoins.

**Qualités requises**
- Un minimum de 3 ans d’expérience connexe.
- Un baccalauréat en services sociaux.
- Excellentes habiletés en communications interpersonnelles.
- Esprit d’analyse, bon raisonnement et aptitudes à la facilitation.
- Bonne compréhension des exigences de coordination des services dans l’ensemble des organismes et ministères.
- Connaissance des services disponibles pour les enfants souffrant de déficience intellectuelle, et de la gestion de cas.
- La connaissance de la Loi sur les services aux personnes ayant une déficience intellectuelle (LDPDI) et de la Loi sur les services à l’enfance et à la famille (LSEF) est essentielle.
- Bilinguisme - préféré, connaissance des deux langues officielles, parlées et écrites.
- Excellentes aptitudes organisationnelles et de gestion du temps pour respecter des délais prévus.
- Aptitude à travailler dans un milieu où les activités se déroulent rapidement et à traiter de diverses questions simultanément.
- Bonne connaissance des logiciels Microsoft Office, y compris Word, Excel, PowerPoint, Outlook et Internet.
- Le titulaire doit avoir un permis de conduire en règle et accès à son propre véhicule pour ses déplacements d’affaires.
