**GENERAL DESCRIPTION**:The Coordinator of Indigenous Community Support and Engagement will identify as an Indigenous person and have connections to Indigenous comminutes and networks across the City. They will develop and provide culturally informed services, support pathways to care, and reduce organizational barriers to culturally safe care for Indigenous community members and TNG-UHHS participants. In addition, the Coordinator will provide culturally appropriate group facilitation and case management for individuals who identify as Indigenous, are living with mental health and substance use challenges. This position is part of **CUPE LOCAL 2289**.

**General Function**:
The Coordinator will conduct group programming which will be developed and facilitated for Indigenous community members using a culturally specific and a harm reduction approach while integrating recognition of intergenerational trauma and the ongoing harms of colonization. Networking and relationship building with culturally specific agencies is needed to ensure partnership development and smooth service transitions/referrals.

The position also provides direct case management to Indigenous adults on a complete range of complex issues, through extensive one-to-one supports to assist participants in identifying and achieving goals. Working closely with the client and external service providers involved, the Case Manager liaises with and coordinates access to services including substance use, mental health supports, landlords, health care providers, legal institutions, income supports, housing providers and community services

**MAJOR DUTIES**:

- Provide Case Management services and support to Indigenous UHHS program participants
- Facilitate and develop TNG-ICSVT curricula for group programming to reflect a combination of harm reduction approaches with cultural supports i.e. Indigenous social determinants of health, knowledge and practices, and

Indigenous Harm Reduction
- Support Indigenous focused outreach efforts in Kensington Market neighbourhood
- Organize and facilitate weekly group and monthly events for Indigenous community members
- Develop, nurture, and support successful partnerships with Indigenous health, social, and community service providers

(e.g., WMS, law enforcement agencies, housing providers etc.), with the goal of enhanced coordination of services of indigenous community member
- In collaboration with the Manager, ensure meaningful client and community participation in the development of programs and services\
- In collaboration with the Manager, develop all administrative and evaluation tools for the program
- Connect program participants with supports in the community that will help them achieve and maintain their self
- identified goals
- With senior leadership, work to seek funding to recruit, train, and support a team of 4 Indigenous Peer Workers

**REQUIREMENTS & QUALIFICATIONS**:

- BA/B.S.W or B.Ed. in Adult Education plus 3 years work experience with individuals who use drugs, experiencing homelessness, and/or mental health issues or a combined equivalent of education and experience, or equivalent combination of education and experience/employment and life experience
- 2 years’ supervisory experience with volunteers, peers or individuals with lived experience
- Must be actively engaged with local Indigenous communities
- Must have solid connections with Indigenous service providing agencies and community networks
- Significant knowledge of traditional teachings and ceremonies
- Demonstrated experience with and commitment to harm reduction principles and practices and ability to integrate these into culturally specific and appropriate group programming
- Demonstrated leadership skills and experience working on a multidisciplinary team
- Recent experience and demonstrated understanding in the areas of substance use, mental health, harm reduction, and homelessness
