- Funded by the Ministry of Community and Social Services, Developmental Services includes a full range of services to adults who are socially isolated and live with developmental disabilities. What You Will Do_**
- _
Develop trust and rapport with clients receiving services through Madison Community Services in order to identify individuals who could benefit from dual diagnosis support and coordinated care coordination._
- _ Provide comprehensive case management on a person directed basis for individuals with a dual diagnosis or who are living with symptoms of a mental illness and/ or addictions._
- _ Assess individual needs, assist in setting goals, and create Coordinated Care Plans unique to the individual._
- _ Provide daily intensive support for Madison Community Services clients in working towards their goals identified in their CCP._
- _ Advocate on an individual basis or with organized groups regarding issues related to the needs of Madison Community Services clients who identify as having a Dual Diagnosis or are experiencing symptoms of a mental illness and/or addictions._
- _ Provide crisis intervention and practical assistance to clients as required in homes and community settings._
- _ Participate in the planning and implementation of program social activities._
- _ Participate with the Program Manager in case management review, program evaluation and annual operational and staff professional development._
- _ Network with other relevant agencies, community groups and/or coalitions regarding issues and needs of clients._
- _ Provide Drop-In support to Madison clients on a weekly basis. _
- _Participate in case consultations and or team meetings as requested/required._
- _ Respond effectively and professionally to clients, situations and emergencies._
- _ Provide crisis intervention as required._
- _ Ensure the organization’s policies and procedures are carried out._
- _ Ensure excellent communication with staff who work on-site at the various specified locations, including Madison Support staff, PSWs, Supervisors and other partners in the community._
- _ Keep current about the range of services available in other agencies. _
- _Provide mentorship, training and/or supervision to student placements and volunteers._
- _ Promote the healthy functioning and participate in team and unit meetings regularly._
- _ Participate in ongoing training and education._
- _ Participate in supervision sessions and annual performance appraisals with program manager._
- _ Maintain appropriate records and documentation in a timely manner and collect statistical data required for the accountability of the program._

What You Bring to the Team_**
- _
Masters of Social Work or Bachelor’s Degree in relevant field. _
- _2+ years of experience in mental health, addictions, and/or social work._
- _ Demonstrated experience facilitating psycho-social groups and supporting individuals with significant mental health and/or addictions issues, and/or Developmental Disabilities from a harm reduction, trauma informed, and mental health recovery approach._
- _ Effective September 7, 2021, all newly hired employees will be required to provide proof that they have completed an approved course of vaccination against COVID-19 at least 14 days prior to commencing employment or provide documentation to support an exemption based on grounds covered by the Ontario Human Rights Act (i.e. Medical exemption)._

- _ Excellent oral and written communication skills and documentation skills._
- _ Self-directed and independent, but also proven experience in working effectively as a team member in collaboration with other community professionals. _
- _Ability to exercise good judgment and flexibility._
- _ Ability to work from a client-centred approach._
- _ Comprehensive understanding of the mental health, addictions, developmental services, health sector, and community resources._
