SJCH provides recovery-based supportive housing in a caring community in partnership with individuals who are living with chronic mental health challenges.

To enable residents to live interdependently in the community in an atmosphere of support, cooperation, mutual respect, and encouragement.


We are seeking dynamic and innovative individual who will work enthusiastically to deliver recovery oriented, client and family centered and comprehensive services that meet client needs, ensure client safety and work within a quality improvement framework to identify opportunities for enhancing quality and quantitative outcomes of the services.

Client and family centered care is guiding principle at SJCH that helps improve decision making processes, health outcome and client experiences.

The successful individual will embed this guiding principle into their work.

About the Position:

The Resource Worker, Dual Diagnosis provides support to clients living with a dual diagnosis (co-occurring developmental disability and mental illness), as well as clients living with symptoms of a mental illness.

The Resource Worker also works with clients who have dual diagnosis and justice system involvement.

She/he is responsible for engaging with each client to provide social and community supports to help them maintain and thrive in their home.

This will include developing goal plans, providing service coordination, coaching, skills teaching, safety planning and linking/referral supports aimed at assisting each client to achieve his/her goals.


Resource Worker must collaborate with clients, client families/natural support networks and agencies/services in the mental health, justice, and developmental services sectors to effectively fulfill their role.


Primary Responsibilities:

  • Reviewing the referral information of applicants to the program.
  • Coordinating and participating in screening interviews with applicants and their respective referral sources.
  • Determining eligibility for the program and confirming acceptance of new clients into the program. Completing a comprehensive assessment of the nature of each client's unique goals and needs through collaborative engagement with each client and their respective support networks.
  • Assist clients in the development and implementation of a care plan tailored to the client's needs and issues designed to improve their quality of life
  • Act as a liaison and client advocate among those involved with the client's care including family, other caregivers, medical and other multidisciplinary providers.
  • Conduct assessments and provide timely and accurate record keeping and reports as required.
  • Provide supportive counselling or referral and linkages to health care, programs, or service providers.
  • Provide crisis management/intervention and assist in the development of crisis plans for clients.
  • Provide flexible and intensive onetoone individual support and followup to the clients and assist them to develop and reach their recovery goals.
  • Assess clients for the need for other services, including nursing, psychiatric, medical, harm reduction, peer support and addictions assessments and treatment.
  • Assist individuals to access a range of services in the broader community.
  • Accompany clients to relevant appointments when needed. Advocate on behalf of homeless individuals in accessing appropriate services such as health care and financial supports.
  • Work cooperatively with other service providers to better meet the needs of clients.
  • A minimum of two years' previous experience supporting people with a developmental disability, mental illness, or dual diagnosis.
  • Previous service coordination experience is necessary.
  • Sound theoretical and practical knowledge of cooccurring developmental disability and mental illnesses.
  • Sound knowledge and understanding of the client population and demonstrated ability to work independently and effectively with individuals with serious mental illness and complex needs.
  • Extensive knowledge of community services and resources relevant to client population will be considered an asset.
  • Demonstrated ability to provide holistic and comprehensive case management services to all clients including initial and ongoing assessments, goal setting, short and longterm care plan development, crisis intervention, progress monitoring, individual money management Knowledge of principles of case management theory and practice.
  • Demonstrated ability to use problemsolving techniques to identify and assess client needs and develop appropriate intervention strategies.
  • Demonstrated ability to engage in a meaningful relationship with a broad range of clients including those that are resistant.
