Position Summary
Social Worker, Adult Mental Health and Transitional Aged Youth (TAY) Program
As a Social Worker, you will be a member of an interdependent multidisciplinary team. You will be responsible for providing individual, family and group therapy using evidence-based therapies such as CBT and DBT and harm reduction and other substance use best practice. The role also involves providing education to students, parents, and teachers at various schools in North York. You have the ability to work independently and, in a team setting, and able to set priorities, organize and coordinate clinical activities. You have experience in providing individual, family and group therapy, and counselling individuals experience substance use related issues. You have demonstrated skills in program development implementation and evaluation and in keeping statistical records. You have the demonstrated clinical ability and the ability to broker and coordinate community resources. You have skills in CBT, DBT, Motivational Interviewing and Brief Therapies. You also have excellent listening and communication skills, both verbal and written. This position supports the Hospital’s academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for staff, physicians, volunteers, learners, patients and families.
On a practical level, you will

Deliver evidence-based therapies and complete thorough assessments for individuals with mental health and/or substance use issues.

Support and adhere to North York General Hospital and Social Work policies and procedures

Perform all aspects of Social Work practice in accordance with professional Standards as set out by the Ontario College of Social Workers and Social Service Workers

Perform Social Work duties as required to meet the needs of the patient/client population

Maintain an updated knowledge of community resources, identify gaps in service and respond to client and community needs

Participate in program planning, implementation and evaluation

Participate in the development and maintenance of effective linkages with Hospital and community service providers. E.g. School Boards, the Courts.

Participate in the education of staff, students and the community

**Qualifications**:
Current registration with the Ontario College of Social Workers and Social Service Workers

Certificate/diploma in Addiction or equivalence

Master of Social Work or equivalent

Membership in good standing or eligibility for membership in the Ontario College of Social Workers and Social Service Workers or equivalent
