Resident Care Coordinator

Possess a DEC in Social Service Technology or Special Education Techniques; Bachelor of Social Work or Psychology, or a related field.
- 2 years minimum experience working in/with mental health, adult population.
- Possess a professional, caring, mature, and responsible work ethic.
- Able to take initiative and be of an open mindset (roll up your sleeves, can-do attitude).
- Must be flexible and able to adapt to changing needs and situations.
- Must be able to intervene and manage mental health crisis situations.
- Work as part of a team (this includes volunteers and members of the Board of Directors) to support residents to live autonomously.
- Conduct intakes, participate in interviews, and assess the appropriateness of potential residents: goal 0% occupancy rate.
- Be familiar with and understand how referral resources, community agencies, and health services function in Québec.
- Accompany residents to medical appointments, being present and advocating for their needs as needed.
- Conduct weekly apartment meetings and follow-ups.
- Liaise with community partners to provide integrated programs.
- Maintain complete, accurate records and reports.
- Provide backup support during vacation or absence of regular Coordinators.
- Assist in planning and overseeing individual service plans and group activities.
- Undertake administrative assignments as requested (i.e., answering the telephone phone and taking messages, preparing monthly activities calendar, preparing weekly resident update summary, filing and documentation/progress notes, etc.).
- Assisting in maintaining the order and cleanliness of the office and activity room (i.e., tidying up after activities and events, putting away dishes, emptying the trash, etc.).
- CPR, AED, and First-Aid training; Crisis Intervention Training for persons working in mental health preferred.
