- Baccalaureate degree in a health-related discipline, such as: Recreation, Human Ecology, Kinesiology, Social Work, Nursing, Health Education, Community Development, Population Health
- Two (2) years of professional experience in a Health Care setting
- Province of Manitoba Class 5 Drivers License, and access to a personal vehicle to provide service within the Prairie Mountain Health region
- Demonstrated leadership
- Demonstrated written and oral communication skills
- Demonstrated experience in initiating changes and improvements, including skills in workplace re-engineering within a Continuous Quality Improvement (CQI) environment
- Ability to prioritize in a changing environment
- Sensitivity to the human and political dynamics of health care management
- Strong organizational, decision making and problem solving skills
- Ability to display independent judgment
- Ability to respect and promote a culturally diverse population
- Ability to respect and promote confidentiality
- Ability to perform the duties of the position on a regular basis

The Health Promotion Coordinator works in collaboration with partners within and outside the Prairie Mountain Health region to assess, plan, implement, and evaluate health promotion strategies specific to the identified health needs of the region’s populations, to influence positive health behaviors and promote community well-being. The Health Promotion Coordinator functions within the Vision, Mission and Values of the Prairie Mountain Health Authority to address the Strategic Priorities.

**RESPONSIBILITIES**:
**Overview**:
Applies knowledge and skills necessary for health promotion practice that includes:

- Applies a population health promotion approach, including determinants of health and health equity, to the analysis of health issues.
- Applies theory to health promotion planning, implementation and evaluation.
- Applies health promotion principles in the context of the roles and responsibilities of population and public health settings.
- Describes the range of interventions available to address population and public health issues.

Partners with communities to conduct a situational assessment for a specific issue that includes:

- Conducts population assessment using existing or collected health data for a specific health issue.
- Accesses and critically appraises evidence (i.e. published and grey literature, systematic reviews, and promising practices) on the health issue and effective interventions.
- Conducts an environmental scan to identify community assets, resources, challenges and gaps.
- Analyzes all data, evidence, and environmental scan findings to develop effective program and policy interventions.

Plans appropriate health promotion programs that includes:

- Develops a plan to implement program goals, objectives, evaluation and implementation steps.
- Develops a program budget.
- Monitors and evaluates implementation of interventions.

Contributes to policy development and advocacy that reflect community needs and regional guidelines and includes:

- Describes the implications of policy options (eg. health, economic, administrative, legal, social, environmental, political and other factors).
- Provides strategic policy advice on health promotion issues.
- Writes clear and concise briefs for health promotion issues.
- Understands the policy making process to assist, enable and facilitate the community to contribute to policy development.
- Adapts policies and programs to reflect the diversity in population characteristics.

Facilitates community mobilization and builds community capacity around shared health priorities that includes:

- Develops relationships and engages in a dialogue with communities based on trust and mutual respect.
- Identifies and strengthens local community capacities to take action on health issues.
- Advocates for and with individuals and communities to improve their health and well-being.

Engages in partnership collaboration that includes:

- Establishes and maintains linkages with community leaders and other key health promotion stakeholders (e.g., schools, businesses, faith groups, community associations, etc.).
- Utilizes leadership, team building, negotiation and conflict resolution skills to build community partnerships.
- Builds and supports coalitions and stimulates intersectoral collaboration on health issues.

Communicates effectively with community members and other professional that includes:

- Provides health status, demographic, statistical, programmatic, and scientific information tailored to specific audiences (e.g. professional, community groups, general population).
- Applies social marketing and other communication principles to the development, implementation and evaluation of health communication strategies.
- Uses the media, advanced technologies, and community networks to receive and communicate information.
