**Team Lead**
**Bridging Service**
**Acute Care Program**

**Position Description**
Reporting to the Manager of the Bridging Service, and working collaboratively with the Acute Care practice leadership group, the Team Lead supports the Manager to anticipate needs of the outpatient clinic over time and collaborates with the clinic’s interprofessional team to provide effective coordination of client/patient-centered care, consistent with CAMH’s vision and strategic directions.

The Team Lead will assume responsibility for coordinating patient flow, supporting treatment planning, leading special initiatives and group discussions, and supporting complex clinical scenarios.

You will support a healthy workplace that embraces diversity, promotes positive team dynamics and reinforces collaborative practice, complies with all applicable standards, requirements and contribute to the development, implementation and evaluation of best-practices, all of which advance excellence in nursing and client/patient care. The position is located at 1051 Queen Street West and will be a regular day shift.

**Qualifications**

**Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code.**

**Please Note**: This full-time, temporary (12 months) position is part of the ONA Bargaining Unit.

**Salary Range**: $41.51 - $58.44 per hour
