Home and Community Care Coordinator, RPN Position (Non-Remote)

**Duties and Responsibilities: Include, but are not limited to**:

- Client intake, creating care plans, and initiating a client/caregiver matched schedule
- Ensuring accurate client information, service plans, and schedules
- Achieving and exceeding client satisfaction
- Working closely with caregivers and team to investigate issues and ensure prompt follow up
- Managing caregiving performance in collaboration with the Director of Care

**Qualifications/Requirements**:

- Current registration with the College of Nurses
- In-Depth experience in Home and Community Care
- Experience in scheduling software such as Alayacare, Procura, or Goldcare
- Be available to be on call (paid) to provide telephone support to clients and caregivers after hours on a rotating basis (approximately 1 week/weekend per 8 week rotation)
- Valid drivers license and vehicle (travel within community)
- Negative TB Screening
- Valid Vulnerable Sectors Police Check
- Covid Vaccination Certificate
- Current CPR/First Aid
