**Primary Responsibilities**:
Under the direction of the Supervisor, Mooka’am Clinic, the Clinical Treatment Assessment Worker will:

- Participate in the intake process for clients referred to the program; conduct an assessment of the individual, determine their daily living requirements or needs and develop a plan of support and recovery, which includes identifying priorities and goals, in conjunction with other professionals and staff.
- Promote the personal empowerment of each individual by building a rapport and through coaching, mentoring and support, enabling each to meet the daily requirements of their living environment; directly assisting with their daily living activities, particularly in the acquisition of life skills in such areas as cultural support, medication management, developing strategies and establishing a plan for maintaining these daily activities.
- Establish a support network of social, family, cultural, medical and psychiatric community links and resources, ensuring that the linkages are maintained through non-intrusive follow-up, reminders and by occasionally accompanying clients to their appointments.
- Advocate on behalf of the client in navigating the social services, cultural services and health care systems and ensure the accessibility and availability of services.
- Promote and enhance the services of the program and the image of the agency as a representative with external agencies and other professionals.
- Assist in the administration of the program by completing and maintaining the required records or statistics such as files, activity sheets, reports, etc. in accordance with established procedures;
- Participate actively as a team member in program delivery by providing holistic support and consultation to colleagues, identifying gaps in service and recommending solutions, participating in meetings, planning and evaluation initiatives, committees, cultural events and activities; orienting new staff; providing other related assistance as required.
- Maintain confidentiality and maintain and update files in accordance with NCFST policies and/or legislative requirements in Ontario.
- All other duties related to the programs mandate as assigned.

Qualifications, Knowledge and Skills:

- A Bachelor’s Degree in Social Work from an accredited university and at least three (3) years of relevant work experience.
- Alternatively, a combination of education and experience will be considered.
- Experience facilitating psychoeducational and/or culturally based groups for men, women and children.
- Ability to implement various therapeutic modalities (i.e, play therapy, expressive arts therapy).
- The position requires an excellent understanding of Intergenerational Trauma, Family Violence, Addictions and Mental Health.
- Clinical counselling skills that are inclusive of child play interventions, working with high risk youth, and family healing circles.
- An understanding of the therapeutic relationship with an ability to negotiate with clients on goals and priorities.
- High level of familiarity in Indigenous culture(s), particularly with respect to the urban Indigenous population.
- Indigenous applicants will be given priority.
- Demonstrated knowledge of child development, family systems, and mental health services.
- Proven knowledge of current privacy legislation and mandatory reporting requirements in Ontario’s child welfare and mental health systems.
- Good oral and written communication skills, especially proven skills in child welfare and/or healthcare oriented communication.
- Ability to contribute to a team approach with respect to effective service delivery, and a demonstrated commitment to incorporating Indigenous traditions, traditional medicines and values.
- Proven ability to liaise with multidisciplinary health practitioners, including traditional practitioners from Aboriginal communities, and child welfare providers.
- Familiarity with electronic client databases and Microsoft Outlook an asset.
- Knowledge of other Indigenous and non-Indigenous support service providers and programs available in the GTA is an asset.
