**Position**: Home Share Coordinator
**Location**: Cloverdale, BC
**Hours**: 35 hours per week, Monday to Friday with the ability to work on some evenings and weekends when necessary (Full-time, Temporary for approx. 16 months)
We are looking for a Home Share Coordinator to join our Sources Home Share team This individual must be dynamic, flexible and possess strong interpersonal skills.

Description of Position
The Home Share Coordinator is responsible for aiding the program manager in the recruitment, operation, support, and ongoing evaluation of the home share program. Our mission is to provide adults with developmental disabilities with a safe and stable home within the community. By carefully matching and providing ongoing support to our clients and caregivers, we can ensure that the home is stable, provides excellent care, and maximizes the quality of life of both the client and
caregiver.
**General Responsibilities**:

- Participates in the development of program outcomes, the preparation of outcome reports, individual program plans and behaviour programs as necessary.
- Ensures that appropriate goals are set for clients and that all members of the team, work towards reaching the goals of the individual program plans.
- Liaises with natural families, caregivers and ministry/CLBC personnel, including coordinating regular review meetings; also includes ensuring all parties are informed of any changes or issues arising, as pertaining to the client's well-being.
- Carries out home studies leading to the recruitment and training of caregivers, whom the coordinator selects and ultimately matches with clients.
- Initiates, renews, and terminates contracts as needed.

**Program Specific Responsibilities**:

- Acts as a resource to the caregivers, the clients, and their families.
- Apprised of Contractor’s scheduling of weekend visits and vacations of clients with natural families.
- Travel to clients home in the community using your personal vehicle
- Makes regular monitoring visits to family care homes and evaluates caregivers on the standards of care.
- Ensures the Home Share program continues to meet or exceed the COA standards, the policies and procedures of Sources, our funding agencies, and our contracts.
- Attends and participates in program meetings as an active contributor, bringing forth ideas and solutions to problems that arise.
- Attends and aids in the facilitation of the Sources Surrey and Delta Social Group as directed.
- Is available to clients and caregivers by phone for emergencies and any unusual situations.
- Responds to reportable incidents following the standards for unlicensed residential facilities.
- Ensures that all operations of the program are in compliance with British Columbia privacy legislation (PIPA) and FOIPA and PIPEDA (federal legislation).
- Ensures that quarterly and annual reporting occurs in a timely matter.
- Continues to try to expand the program by working with the Home Share Manager to respond where appropriate to funder RFP’s or direct award process.
- Serves on Sources Interdisciplinary Committees where required. Ensures program representation on committees, as required.
- Participates in quality improvement and evaluation processes.
- Other duties as directed.

**Qualifications**:

- A degree or diploma in an area related to supporting people with developmental disabilities (Social Services, Human Services, or equivalent)
- Effectively utilize Office suite including Excel, Word, Outlook and central data systems
- Effectively communicate professionally, verbally and written in English
- Standard First Aid and CPR (Training may be accessed through Sources)
- Non-violent Crisis Intervention Training (Training may be accessed through Sources)
- Criminal Records check
- Valid Class 5 BC Driver's License and driver's abstract

**Start Date**: January 20, 2024
**End Date**: Temporary full-time, 16 months approx. (Dependent on return of employee).
**Salary**: $27.81
