Windsor Essex Community Health Centre (weCHC) is expanding the services available to African Canadian & Caribbean Youth through its partnership with the Centre for Addictions and Mental Health (CAMH) and its Substance Abuse Program for African Canadian & Caribbean Youth (SAPACCY). The primary focus of the Social Worker is to work closely with Black youth dealing with co-morbid addiction and mental illness.

**CORE RESPONSIBILITIES OF THE POSITION**

As a key member of the inter-professional team, the Social Worker will:

- Work largely with Black youth (ages 12-29) (and clients’ families/support systems as appropriate), who are dealing with comorbid addiction and mental illness and work closely with other professionals serving this population
- Deliver care within a model that recognizes the lived experience of Black youth who have faced oppression and racism as it relates to many aspects of their lives inclusive of the social determinants of health eg: housing, employment and education
- Recognize the resiliency and strength, cultural diversity and intersectional experiences of Black youth
- Provide clinical interventions including, outreach, assessment, case management, individual and group therapy and meet clients where they are at, work from a healing centered and holistic model of care
- Practice and operate within an anti-racism, anti-oppression, and strength-based framework
- Support the family and service network while maintaining a client directed philosophy and working within the social work scope of practice, ensuring that professional boundaries are discussed and respected
- Monitor, regularly review and revise service plans with clients and supports
- Provide culturally responsive counselling to reduce the incidence, duration and intensity of crisis that is recovery oriented Africentric and affirms a strong sense of identity
- Support program development and clinical evaluation and be an active participant in continuous quality improvement and research activities
- In coordination with the SAPACCY Case Worker develop working relationships and rapport with community service providers in providing culturally adaptive language and services which consider the experiences of the African, Caribbean, and Black (ACB) community
- Contribute to chart documentation, chart reviews, case conferences as per college and weCHC standards
- Provide flexible and timely services in the client’s environment of choice ensuring safety and privacy for all
- Inform clients (and their family/network) about services available that are most likely to be culturally adaptive
- Assist clients (and families) with advocacy
- Collaborate with community partners to support population health from a social work perspective
- Perform other duties that support the mission and vision of the organization and/or as assigned based on position responsibilities and/or site requirements.

**SKILLS, KNOWLEDGE & EXPERIENCE REQUIRED**
- Minimum of 2 years experience in a community health setting; working in mental health and addictions
- Demonstrated skills and experience in working with African Canadian and Caribbean youth
- Lived experience as a member of the Black community. Demonstrated experience as a role model is essential.
- Knowledge of community resources for African Canadian and Caribbean youth in substance abuse and/or mental health treatment is essential.
- Clean driver’s abstract, current, valid Class “G” Driver’s License and reliable transportation to support travel across Windsor-Essex County and the province, as required
- Bilingual (English/French) language skills an asset

**Education and certificate requirements**
- Graduate degree in Master of Social Work (MSW) required from a recognized University
- Formal training in addictions and/or mental health with two or more years of clinical experience in the treatment of concurrent disorders
- Member in good standing and current registration with the Ontario College of Social Workers and Social Service Workers required.

Pay: $37.70-$45.42 per hour
