Coordination des services pour les personnes ayant une déficience intellectuelle (SCS) est un organisme bilingue à but non lucratif, géré par un conseil d’administratif composé de bénévoles, qui travaille avec et pour des personnes ayant une déficience intellectuelle et leurs familles dans la région d’Ottawa. SCS offre des services aux adultes et aux enfants en leur fournissant des renseignements, des services d’aiguillage et un soutien de gestion de cas. L’un des buts importants de l’organisme consiste à habiliter les personnes/familles à faire des choix éclairés concernant les soutiens qu’ils recherchent.

Relevant directement du superviseur des services aux adultes, le gestionnaire de cas est responsable de la coordination des services aux adultes admissibles ayant une déficience intellectuelle avec la participation de la personne/famille. Au moyen du plan de services, le gestionnaire de cas offre des renseignements, aide à obtenir l’accès aux services et aux soutiens non financés en remplissant au nom de l’adulte des recommandations auprès des ressources communautaires demandées, conformément à la loi et aux protocoles de l’organisme le cas échéant. Le gestionnaire de cas informe les personnes/familles et crée les conditions favorables pour qu’elles deviennent le plus autonomes possible dans leur capacité d’exécuter les mesures décrites dans leur propre plan de services.

**Principales responsabilités**
- Accomplir la fonction d’accueil et toutes ses exigences en matière de documents connexes selon le cas.
- Si une participation continue est nécessaire, le gestionnaire de cas élabore, conformément à l’évaluation des Services de l’Ontario pour les personnes ayant une déficience intellectuelle de la région de l’Est (SOPDIRE) et avec la participation de la personne/famille/réseau de soutien, un plan de service centré sur la personne afin d’aider l’adulte à obtenir les services et/ou les soutiens offerts dans la collectivité.
- Faciliter la planification de la transition des personnes de 18 ans et plus avec la collaboration des personnes/familles, des partenaires communautaires et des écoles.
- Aider la personne/famille en lui fournissant des renseignements et un soutien sur l’aiguillage afin qu’elle puisse avoir accès aux services et aux soutiens appropriés pour l’adulte dans la collectivité.
- Coordonner les conférences de cas et y participer avec la personne/famille et/ou d’autres organismes afin d’appuyer la personne/famille selon les besoins.
- Agir en défenseur au nom des clients et de leur famille.
- Assurer l’exactitude et l’intégrité des renseignements contenus dans les dossiers et la base de données de l’organisme conformément aux normes établies ; fournir des intrants/données et des analyses statistiques au besoin.
- Participer à des examens mensuels de cas et à des comptes rendus sur l’accueil avec le superviseur des services aux adultes pour obtenir un soutien clinique et recevoir des directives au besoin.
- Évaluer les dossiers et recommander au superviseur des services aux adultes lorsque les services de gestion de cas ne sont plus nécessaires.
- Participer aux initiatives du système se rapportant aux services aux adultes.
- Promouvoir SCS en participant aux exposés de l’organisme pour les groupes de parents, les écoles et d’autres organismes selon les besoins.
- Suivre et appliquer les politiques et les procédures ainsi que les protocoles connexes.
- Exécuter d’autres fonctions selon les besoins.

**Connaissances et compétences**:

- Un minimum de trois ans d’expérience connexe progressive.
- Un baccalauréat en services sociaux.
- Excellentes habiletés en relations interpersonnelles.
- Devoir posséder de solides compétences en analyse, en raisonnement et en animation.
- Solide compréhension des exigences de la coordination des services dans l’ensemble des différents organismes et des ministères.
- Connaissance des services offerts aux adultes ayant une déficience intellectuelle ainsi que de la gestion de cas.
- La connaissance de la Loi de 2008 sur les services et soutiens favorisant l’inclusion sociale des personnes ayant une déficience intellectuelle, du Règlement 299/10 et de la Loi sur les services à l’enfance et à la famille est exigée.
- D’excellentes compétences en communication verbale et écrite tant en français qu’en anglais et de solides compétences en rédaction d’affaires sont préférées.
