**There is one Part-Time contract Clinician, Senior Services position available starting on January 13, 2025, and ending on January 12, 2026. working 7.5 hours per week.**
**Your Role**:
Meets with residents who require counselling services to support with transitions, loss, and other changes impacting their wellbeing. Works as part of an interdisciplinary team to ensure inclusive health and wellness services are available at the Village. Participates in the assessment, design, and implementation of inclusive therapeutic programs. Communicates with resident families to provide resources and other information to support the resident’s wellbeing. Intentionally creates psychologically safe spaces to support residents and families in times of changing care needs. Seeks opportunities to develop, source, and leverage external community partnerships to provide a variety of meaningful resources that meet the holistic perspectives of wellness for residents. Remains informed as to health and wellness trends impacting seniors. Applies an anti-racist, anti-oppressive and inclusive lens to service delivery.
**Tasks**:
Resident Service:
- Meets one-on-one with new residents to provide an orientation of the counselling services available, and initiates discussion on support with transition.
- Facilitates inclusive group counselling sessions on a variety of topics to facilitate meaningful relationships.
- Conducts intake assessments and establishes goals to be achieved through the counselling sessions.
- Participates in monitoring, evaluating, and revising treatment plans.
- Participates in the provision of crisis intervention with residents and families and assesses a resident’s risk of harm to self and others.
- Communicates with the care team where applicable to ensure they are aware of important changes impacting a resident’s mental health.
- Liaises with external agencies for referral and service coordination purposes.
- Provides clinical consultation, direction, and evaluation to care team members.
- Acts as a resource to residents and families in need of support.
- In collaboration with the wellness team, develops and facilitates information sessions on various mental health-related topics to residents and their families.
Administration:
- Books individual and group counselling sessions.
- Regularly updates Point Click Care to ensure multi-disciplinary teams have relevant information while adhering to confidentiality protocols.
- Identifies and reports any incidents that have legislative reporting requirements.
- Internally promotes counselling services available and provides content for social media purposes.
- Provides leadership with statistical information on counselling services offered each quarter.
- Participates in and/or delivers staff training and development.
Other:
- Participates in supervision with Manager, Wellness Services.
- Demonstrates anti-racism and anti-oppression values in all aspects of role.
- Represents Lutherwood in the community.
- Participates in Agency committees and task forces.
- Complies with the policies and procedures of the Agency as outlined in the Policies and Procedures Manual.
- Complies with the duties imposed by law or contract and the policies and procedures for performing the job in a safe and healthy manner.
- Takes an active role in promoting and protecting personal health and safety and the health and safety of others, both staff and consumers. (Sec.28(1)OHSA)
- Performs other duties as assigned.
**Your Talents and Experience**:
Holds a relevant degree in psychology, social work or related discipline at a master’s level, and a minimum of two years’ experience in a multidisciplinary treatment setting. Must maintain membership within their regulatory college and professional association. Experience with dementia care and gentle-persuasive approach preferred. Highly adept at assessing and providing treatment to older adults and their families experiencing challenges related to health, cognitive decline, end of life, mental health, and trauma.
