**Research Coordinator I**
The Compass Clinic in the Department of Pediatrics serves children and youth who are new to Canada with special health care needs, including chronic illness, developmental diagnoses and psychosocial conditions. The clinic uses an intersectional and holistic approach to medical care for chronic diseases by considering the social support that is needed by families that are new to Canada. Children who are new to Canada with chronic conditions face many more challenges navigating between multiple medical settings, school and community services.
**Here’s what you’ll get to do**:
We are seeking a highly motivated, organized, respectful, results-oriented Research Coordinator I to provide support to migrant (immigrant and refugee) families navigating transitions in care. Reporting to the PI/Program Manager, you will be responsible for providing quality patient care for the NAVIGATE trial, a randomized controlled trial examining the impact of a community-designed intervention to support immigrant and research families with special healthcare needs.
The Research Coordinator will assist in:
- Connecting families to health, social or community resources that best fit their child’s needs
- Create a list of resources for immigrant refugee families with special healthcare needs
- Help families attend appointments and meetings
**Responsibilities**:
- Connect families to health or community service resources in the community that are adapted to the relevant needs of refugee and immigrant families (i.e., completing disability funding forms; culturally relevant autism services);
- Facilitate connections between services in an efficient manner while considering the needs and other responsibilities of the families;
- Facilitate information sharing between the patient/family and health care professionals in the hospital and community (ie. acting as liaison, care plans)
- Maintaining a list of programs and services relevant to migrant and refugee children and youth ;- Provides case management support to clients to assist them in meeting their care plans and provide support in times of crisis;- Provides outreach support to clients to help access resources, connect with agencies and health care providers and improve community connections/integration;- Fosters relationships with internal and external partners;- Provides outreach services to clients who are unable to attend office based appointments;- Builds partnerships with community agencies including developing and implementing referral pathways;- Acts as a liaison with the community agencies to ensure appropriate client referrals;- Acts as an advocate on behalf of the registered client when deemed necessary;- Engages in day-to-day liaison activities with relevant professionals and community members;
- Providing support to families to understand the healthcare and social service systems in Ontario;- Records all Client interactions in an objective and accurate manner that reflects organizational protocols and established regulatory practices;- Document all stages of the care transition and coordination process clearly, accurately and concisely;- Monitors the types of services provided for patients and
- families and encounters with the families;- Collects data as needed, e.g., workload statistics, volumes
- , referrals, types of needs identified, wait times,
- etc.;- Maintains project documents, as required.- Conducting community visits, including home visits, as required;
**Qualifications**:
- Undergraduate Degree and 2 years of relevant experience, OR demonstrable equivalent combination of specialized education and experience- Post-secondary qualification in: Public Health, Social Work, Community Health, Community Organizing, Social Service Work or a related discipline; or equivalent community experience, preferred- Minimum 2 years¿ experience in community services or health care, community organizing or community health- Must have extensive knowledge of community service resources for children in the community.- Previous lived or working experience with multicultural communities is an asset;- Must have experience in engaging people facing significant barriers to accessing health and social services;- Strong organizational, coordination and administrative skills;- Previous experience in practicing culturally safe and/or trauma-informed care- Ability to work independently and as part of a multidisciplinary team;- Excellent written and verbal communications skills;- Experience working with diverse communities with complex psycho-social needs- An understanding of the impact of the social determinants of health and a commitment to working collaboratively with patients to mitigate these;
