**Job Description**:
***:Under the direct supervision of the Administrator, the Behavioural Resident Wellness Worker (RWW), will be a part of the Pioneer Ridge embedded Behavioral Support Ontario team and will work under the direction of the Behavioural Support Lead.
In this specialty role, the Resident Wellness Worker liaises with the Pioneer Ridge Interdisciplinary Team, Behavioral Support Team and Behavior Support Ontario to ensure the social, emotional, cultural and spiritual needs of all residents exhibiting responsive behaviours/personal expressions, are individualized, successful, and resident centred.
**MAJOR RESPONSIBILITIES**:
- Taking a person-centred approach, assesses the reason behind an individual's challenging behaviours related to health and wellness and develops plans and strategies to reduce the behaviour and improve the person's quality of life.
- Provides coaching, care planning, monitoring and evaluation of all Emotional and Well Being interventions that cover the physical, spiritual, artistic, intellectual, social, and sensory domains.
- Reports all findings to the Behavioural Support Lead and follows the guidance provided by the Lead.
- As a multidisciplinary team member in a specialty role, ensures safe and holistic programming for residents presenting with, or at risk for, responsive behaviours while ensuring residents utilize maximum levels of independence and choice.
- Acknowledges that responsive behaviours may be associated with dementia, complex mental health, substance use and/or other neurological conditions.
- Participates in care conferences and/or Responsive Behaviour rounds, as appropriate.
- Assists staff who are working with residents, to use frameworks such as P.I.E.C.E.S and UFirst to assess responsive behaviours/personal expressions, identify causes, formulate, and implement coordinated care plans and therapeutic interventions.
- Assesses for aspects of risk and intervenes as appropriate.
- Documents all findings and interventions in a clear and concise manner in the resident’s medical record and completes other reports as required.

- Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.
- Provides hands on patient support, along with data collection and care planning; provides direction, reporting and required documentation.
- Effectively uses empathy in interactions with others and demonstrates behaviours consistent with Pioneer Ridge’s Code of Conduct.
- Documents all findings and interventions in a clear and concise manner in the resident’s medical record and other reports as required.
- Models behaviour that is aligned with the values of Pioneer Ridge - Providing Premier Services.
- Participates as a member of the Pioneer Ridge Behavioural Support Team to provide an exceptional experience for staff, families and residents within a challenging and exciting health care environment.
- Performs such other related duties as may be required.
**QUALIFICATIONS**:
**Education/Experience**:
- Degree or diploma in Gerontology, Developmental Social Work, Social Work, Recreation & Leisure Studies, Therapeutic Recreation, Kinesiology or a related discipline;
- Two years’ relevant clinical experience in long-term care
**Skills/Abilities**:
- Ability to foster cooperative and collaborative working relationships with inter-disciplinary teams, residents, family members and community team members
- Ability to exercise independent judgement when dealing with confidential or sensitive issues
- Ability to communicate effectively and concisely, both orally and in writing
- Must be able to deal with the public in a courteous and tactful manner
- Excellent organizational and time management skills with flexibility and adaptability.
- Must be proficient in the use of computers and current related software
- Must be proficient with Electronic Health Records (Medccare, Point Click Care, Surge Learning)
- Ability to implement best practice in dementia, delirium, mental health issues and their effect on persons, families and caregivers, based on scope of practice.
