**KEY FUNCTION**:
Implement the geriatric standard of care using Alternative Level of Care (ALC) Leading Practices, the Senior Friendly Care Framework, and the Geriatric Rehabilitative Care Best Practice Framework. Teach and mentor front-line clinicians in geriatric care (i.e.early identification, assessment, care planning, interventions, and transitions). Collaborate with health service providers to develop and enhance models of care, ensuring equitable, high-quality care for older adults across the North East.
**REPORTING**:
Under the general direction of the Regional Clinical Quality Lead, North East Specialized Geriatric Centre.
**DUTIES**:
- Lead the implementation of the geriatric standard of care within Specialized Geriatric Services (SGS) and across systems of care for older adults; implement guidelines, frameworks and evidence using tailored and innovative approaches to meet the unique needs of each community.
- Demonstrate, teach, and mentor front-line clinicians in the geriatric approach to care (i.e.early identification, assessment, care planning, interventions, and transitions).
- Develop and action local implementation and evaluation frameworks; collect, monitor and report key performance indicators and targets within the subregion.
- Maintain a sub-region asset inventory contributing to the regional asset map; report to the Provincial Specialized and Focused Geriatric Services Asset Inventory maintained by Provincial Geriatric Leadership Ontario (PGLO).
- Collaborate in the research, development, validation, and implementation of clinical tools, resources, models, and approaches to care.
- Implement outcomes of the SGS System Steering Committee at the local level.
- Ensure a standardized approach is employed to the implementation, monitoring and evaluation of geriatric rehabilitative care best practices, implementation of Rehabilitation Care Alliance (RCA) / PGLO best-practice recommendations, and clinical quality measurement within the sub region.
- Collaborate with Ontario Health and local resources to implemement, monitor and optimize geriatric care across the continuum within the sub region.
- Advance and advocate for integrated care for older adults in collaboration with networks and partners.
- Include older adults, families and caregivers in strategic and operational activities.
- Administer and perform Comprehensive Geriatric Assessment and treatment in line with the expectations outlined within the ‘A Competency Framework for Interprofessional Comprehensive Geriatric Assessment’; include the analysis and synthesis of findings into a clinical report identifying client’s priority needs and relevant treatment options designed to optimize their independence.
- Administer and implement Comprehensive Geriatric Assessment model of care in the community, Emergency Department, acute and post-acute environments.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.
**QUALIFICATIONS**
**EDUCATION AND TRAINING**:
- Minimum of a four (4) year Bachelor’s Degree in a health related field, from an accredited university.
- Master’s degree in a health-related field is preferred.
- Current Certificate of Registration in good standing as a Health Professional with an Ontario Professional Association or regulatory college.
- Advanced training in specialized geriatrics is required.
- Successful completion of the PGLO Provincial Common Orientation or equivalent.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
**EXPERIENCE**:
- Minimum of five (5) years’ experience working in specialized geriatrics and completing assessment and treatment guided by the Comprehensive Geriatric Assessment (anywhere across the care continuum).
- Experience in process improvement, change management and project planning is required.
**KNOWLEDGE/SKILLS/ABILITIES**:
