Unity Health Toronto is embarking on a multi-year digital, clinical transformation project to implement a new electronic patient record solution across all sites. The Electronic Patient Record (EPR) project is a pillar of enabling exemplary patient experience and quality of care and therefore see this not as an information technology initiative, but as one founded in clinical transformation and care redesign.

**Job Profile**:
The project manager patient partnerships EPR ensures that the patients and families are actively involved and heard throughout the EPR program and that the EPR solution and future state workflow reflect their needs. The project manager patient partnerships EPR also support wider project management activities across the change management and education team. They have both patient experience and project management expertise.

**Main Responsibilities**:

- Planning and developing all aspects of large scale projects related to engagement of patients and families along with patient and family education and resources;
- Leading implementation of activities related to engagement of patients and families along with patient and family education and resources for the EPR project;
- Leading project management activities across the change management and education team including the development and monitoring of plans, reporting, documenting, dependency monitoring and capturing decisions, risks and issues;
- Managing a group of 20+ Patient and Family Partners (PFPs), including recruitment, resources allocation and acting as primary liaison for PFPs;
- Educating project teams, leaders, clinical teams and other key stakeholders and working groups on how to engage PFPs;
- Ensuring quality standards are met;
- Working with patient and families to co-design solutions and delivering insights to enable project delivery;
- Developing, implementing and planning key patient, resident, caregiver and community engagement strategies to inform a patient and family centered approach to the development and implementation of the new EPR;
- Identifying and implements measures to access the success of patient and family engagement and ensure that the program reflects their needs;
- Leading the collaboration with relevant parties in the development of evidence based measurement and metrics to benchmark performance against industry standards and peer organizations.

**Qualifications**:

- Completion of a recognized Bachelor’s Degree required. Completion of a Master’s Degree preferred;
- Minimum 8 to 10 or more years’ of project management experience in a health care environment required;
- Project Management Project (PMP) certification required;
- Demonstrated excellent experience in facilitation, collaboration and negotiation with multiple stakeholders/service providers required;
- Demonstrated experience and understanding of patient and family and community engagement required;
- Demonstrated experience generating patient and caregiver insights;
- Demonstrated understanding of co-design principles;
- Excellent verbal and written communication skills with the ability to communicate information/ideas, in a manner that is easily understood by others required;
- Excellent customer service, interpersonal and listening skills with the ability to effectively interact with staff all levels required;
- Excellent computer skills e.g., Microsoft Word, Excel and PowerPoint, required;
- Excellent problem recognition and problem solving skills required; with the ability to deal with situations where information is difficult to obtain, complex or ambiguous required;
- Detail oriented with excellent analytical skills related to proposal development/project management required;
- Ability to effectively analyze complex problems, interpret operational/clinical needs and develop integrated, creative solutions required;
- Ability to solve complex problems independently required;
- Ability to multi task while producing high quality work required;
- Ability to work efficiently and effectively as a team player required;
- Ability to utilize a high level of initiative and self-direction, required;
- Ability to manage multiple and often complex projects simultaneously required;
- Ability to effectively handle situations where information is difficult to obtain, complex or ambiguous required;
- Ability to work in a fast pace environment and motivate project staff and others to achieve a common goal required;
