Program Overview - Adult Development Services, Community Care_**
- Funded by the Ministry of Community and Social Services, Developmental Services includes a full range of services to adults who are socially isolated and live with developmental disabilities. These services include case management support, linking to daily living supports and community support services._

What You Will Do_**
- _
Provide comprehensive case management on a person directed basis for Adult Protection Service Worker (APSW)._
- _ Assess individual needs, assist in setting goals and creating Individual Support Plans (ISP)._
- _ Provide flexible support to APSW clients in accomplishing the ISP._
- _ Provides specialized services and interventions related to Dual Diagnosis including evidence-based practices, and collaboration with external agencies and resources._
- _ Advocate on an individual basis/with organized groups regarding issues related to client needs. _
- _Participate in the planning and implementation of program social activities. _
- _Maintain individual case files according to Agency and Ministry of Community and Social Services (MCSS) policies and standards._
- _ Participate with the Manager in case management review, program evaluation and annual operational and staff development planning. _
- _Participate in and support the work of the APSW Network (Toronto APSAO chapter)._
- _ Network with other relevant agencies, community groups and/or coalitions regarding issues and needs of consumers._
- _ Participate in case consultations as requested/required._
- _ Respond effectively and professionally to clients, situations, and emergencies and provide crisis intervention as required._
- _ Maintain compliance with MCSS directives and Quality Assurance Measures (QAM) requirements._

What You Bring to the Team_**

BSW or Bachelor’s degree with a related major. _
- _2+ years of experience working in developmental or dual diagnosis services. _
- _Experience working with high-risk, marginalized, and vulnerable populations, including those with developmental disabilities._

- _ Creative conflict resolution/mediation and crisis intervention skills._
- _ Innovative and resourceful._
- _ Experience working with diverse communities._
- _ Experience working with concurrent disorders._
- _ Knowledge of resources in Toronto (both generic and specialized)._
- _ Valid First Aid and CPR Certificate._
- _ Community mental health experience._
- _ Experience supporting individuals with Dual Diagnosis considered an asset._
- _ Specialized training in trauma response, suicide risk assessment/management or bereavement counselling._
- _ Verbal and written fluency in French is an asset._
