Touchmark is Edmonton’s leading full-service retirement community, enriching the lives of residents and their families through five-star hospitality, services and amenities. We are currently seeking a Social Worker to join our Long Term Care Health Services Neighborhood.

As a Social Worker, you will have the opportunity to distinguish yourself by taking the time to be a positive presence, provide ongoing assessment of the resident needs and work with residents, families, and staff in promoting their psychosocial wellbeing.

This position is 18.75 hours per week.

**Some responsibilities include, but not limited to**:

- Assess the psychosocial status of residents and families/caregivers related to the residents’ illness and environment. Communicates findings to the registered nurse and other members of the interdisciplinary group;
- Carry out social evaluations, including family dynamics, caregiver abilities, neglect or abuse and plans intervention based on evaluation findings;
- Counsel patient and family/caregivers as needed regarding stress and other identified coping difficulties. Provides crisis intervention when necessary.
- Maintain clinical records on all patients referred to social work;
- Provide information and referral services for patients and families/caregivers regarding practical and environmental needs;
- Participate in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings
- Possess a Bachelor of Social Work Degree
- Have an active registration with the Alberta College of Social Workers (ACSW)
- Experience working in a Long Term Care environment
- Possess good stress management and crisis intervention abilities
- Have a genuine interest in caring for elderly residents and working in a geriatric environment
- Be able to maintain patient and tactful composure when dealing with residents, family members, staff members and visitors
