**Details**: This position will assist three (3) adults with developmental disabilities, complex mental health diagnoses, complex behaviors, high risk issues and substance abuse. The adults require support to foster community relationships, daily living skills, decision making using a best practice approach to help transition to a greater degree of independence and healthy lifestyle choices.

**Hours**:Saturday, Sunday, Monday, Thursday - alternate 2 week periods of mornings from 7:00am-3:00pm and evenings 3:00pm-11:00pm; total of 32 hours per week

**Responsibilities include**:
¨ Acting as a strong advocate for the individuals in the community

¨ Assisting individuals with daily living skills and finding employment

¨ Assisting with substance abuse issues using a Harm Reduction approach

¨ Assisting with managing mental health, substance use, health issues and trauma with other community partners in a multidisciplinary team

¨ Behavior Management/supporting adults with complex needs; support with legal matters

¨ Program Implementation and development

¨ Case management, including maintaining and updating working files

¨ Completing daily documentation and data collection

¨ Working with individuals and staff team to create, implement, maintain and update personal support plans

¨ One year of post secondary education in a related field

¨ One year of experience working with adults with mental health and addiction concerns; demonstrated case management experience

¨ Equivalencies may be considered

¨ Security Clearance Check; including Vulnerable Persons Sector Check (within the last 6 months)

¨ Emergency First Aid-Level A CPR + AED

¨ Valid Class 5 drivers license, drivers abstract, reliable vehicle and auto insurance **are required; ability to drive an agency vehicle is an asset.**

¨ ¨ Familiarity working with high risk populations, addictions, poverty, houselessness, institutional backgrounds, and complex mental health diagnoses

¨ Ability to build healthy relationships and manage crisis situations

¨ Excellent verbal and written communication skills; computer/word processing skills

**Starting Rate of Pay**:$27.03-$28.68 per hour; depending on experience and education
