Sherbourne provides innovative health care and social services to the diverse urban population of southeast Toronto. While our doors are open to everyone, we focus on people who are homeless or under-housed; 2SLGBTQ, and newcomers to Canada. We are funded by the Ministry of Health, Ontario Health Toronto, Ministry of Community, children and Social Services, City of Toronto and many generous companies, groups, and individuals.
The St. James Town Community Corner (The Corner), a multi-service, inclusive hub governed by residents and service providers, is a unique, integrated service model between 20 partner organizations. The Corner is the primary hub for Health Access St. James Town (HASJT) intake, referrals and service coordination. HASJT is a neighborhood-based client-focused model of integrated, collaborative care through inter-sectoral service coordination, service enhancement and community development. This initiative is a collaborative between 15 health and social service organizations that form the Health Access Executive Table. The above is a community centered initiative aimed to enhance interdisciplinary care including primary care, improve access to health, social and community services in St. James Town (SJT), and promote health and wellbeing in the community. Thorncliffe Neighbourhood Office and Sherbourne Health Centre co-lead this integrated service model comprising The Corner and HASJT. This model is seeking to hire a full-time case worker who will work as part of the intake, referral and service coordination team.

The Client Engagement Worker is a team player and an integral member of The Corner, responsible for greeting clients, ensuring they feel welcome, and providing information about The Corner and partner agencies' services. This role will support and provide culturally sensitive services with an anti-oppressive framework. This role will be working in a fast-faced environment with a multidisciplinary team to provide services.
**day to day responsibilities**:
- Registration of all new clients at The Corner and in the community, satellite sites to ensure the client navigates and access the services with ease in a safe and respectful manner.
- The position also works closely with the Holistic Intake & Navigation function at The Corner and updating information on new/existing clients in the database and clarify eligibility criteria.
- Engage proactively with residents, clients and service users, and will be responsible for supporting staff and partners with crisis intervention and de-escalation as needed.
- Good organizational skills to ensure a smooth flow of clients to the different services on site and to work closely with the intake staff to ensure all residents seeking services are connected and directed as appropriate.
