- Le CIUSSS de l'Ouest-de-l'Ile-de-Montréal est à la recherche d'un(e) chargé(e) de projet, pour combler un remplacement de congé de maternité à temps complet, à la direction des services multidisciplinaires - Volet psychoéducation - Centre de protection de l’enfance et de la jeunesse (CPEJ), Centre de réadaptation pour jeunes en difficulté d’adaptation (CRJDA) et Jeunes en difficulté (JED). L’emploi est basé au CLSC de Dorval-Lachine. La personne retenue sera appelée à se déplacer sur l’ensemble du territoire du CIUSSS.
- Titre d’emploi à l’interne : Agent(e) de planification, de programmation et de recherche (APPR)
- Sous l'autorité du conseiller cadre en réadaptation sociale, le/la titulaire de ce poste contribue à l'amélioration et au développement des bonnes pratiques ainsi qu'à l'élaboration des programmes. L'APPR soutient l'élaboration des programmes et participe au développement d'outils cliniques et à leur implantation. Il/elle soutient les intervenants et gestionnaires sur le plan clinique et participe à l'évaluation des besoins de formation.
- Dans votre champ d'expertise clinique, vous soutiendrez et dispenserez des formations aux intervenants incluant des spécialistes en activités cliniques et des coordonnateurs professionnels, afin d'assurer le rehaussement des compétences du personnel dans votre secteur d'activité. Vous contribuerez à développer et assurer la qualité des pratiques professionnelles en s'appuyant sur les données probantes et les meilleures pratiques et ce, pour l'ensemble des professions représentées au Conseil multidisciplinaire de l'établissement.
- Dans le cadre de votre mandat spécifique, vous soutiendrez la pratique des psychoéducateurs et éducateurs spécialisés œuvrant auprès de la clientèle jeunesse, incluant le volet réadaptation en protection de la jeunesse et jeunes contrevenants.
- Horaire : lundi au vendredi, 8h et 18h, flexibilité d'horaire avec possibilité de temps partiel (disponibilité mínimale de 3 jours/semaine)
- Dans le cadre de vos fonctions, vous serez appelé(e) à:
- Fournir un support clinique aux équipes d'intervenants et aux gestionnaires;
- Élaborer différents outils cliniques pour soutenir la pratique professionnelle (ex : aide-mémoire cliniques divers, fiches);
- Entretenir des liens de collaboration avec les ordres professionnels pour assurer une conformité aux meilleures pratiques attendues;
- Participer à l'élaboration et à la révision des structures de contrôle de la qualité;
- Préparer et dispenser divers ateliers cliniques en lien avec les besoins de formation des directions cliniques;
- Participer à l'élaboration des profils de compétences avec les directions impliquées;
- Participer au développement des outils de dotation en collaboration de la direction des ressources humaines;
- Élaborer, mettre en place, réviser ou coordonner le déploiement d'un modèle, d'une approche ou d'un programme d'intervention en collaboration avec les directions cliniques;
- Participer à l'élaboration et réviser différents documents qui encadrent la pratique tel que : politiques, procédures, protocoles, cadres de références, guide d'intervention, etc.;
- Valider des documents cliniques élaborés par les directions cliniques en vue de s'assurer de la conformité en lien avec le cadre législatif ainsi que les politiques et règlements en découlant.
