Case Manager Job Responsibilities:
- Accomplishes clients’ care by assessing needs; developing, monitoring, and
evaluating plans and progress; facilitating interdisciplinary approaches.
- Admits new clients by reviewing records and intakes; conducting orientations.
- Determines clients’ requirements by completing intake interviews; determining
needs for community referrals.
- Establishes individual journeys by setting schedules and routines; coordinating
services being provided; arranging resources, including transportation in
conjunction with Peer Support.
- Monitors cases by verifying clients’ attendance; observing and evaluating
treatments and responses; advocating for needed services and entitlements;
obtaining additional resources; intervening in crises; providing personal support.
- Maintains clients’ records by reviewing case notes; logging events and progress.
- Communicates clients’ progress by conducting weekly interdisciplinary meetings
and evaluations; disseminating results and obstacles to the clients team.
- Prepares clients’ discharge by reviewing and amplifying discharge plans;
coordinating discharge and post-discharge requirements; providing resources.
- Improves treatment results by studying, evaluating, and re-designing processes;
implementing changes.
- Collect statistics on departed clientele and report it to Transition House
Coordinator.
- Updates job knowledge by participating in educational opportunities; reading
professional publications; maintaining personal networks; participating in HART.
- Enhances department and organization reputation by accepting ownership for
accomplishing new and different requests; exploring opportunities to add value
to job accomplishments.

Education, Experience, and Licensing Requirements:
- 3+ years of case management experience, preferably in human services, possess
demonstrated commitment to clients needs.
- Professional or personal experience supporting individuals with disabilities,
mental illnesses, or challenging behaviors highly preferred
- Must have a valid driver’s license, provide current registration and insurance.

Salaire: $52,000 annuellement
Tâches principales:
- Effectuer des interventions en cas de crise ;
- Rencontrer avec les clientes individuellement pour déterminer leurs besoins et les aider
à identifier leurs objectifs à court et à long terme;
- Organiser des sessions de groupe;
- Orienter les clientes vers les services communautaires si nécessaire;
- Engagement communautaire continu;
- Tenir à jour les dossiers des clientes en consultant les notes de cas, en notant les
événements et les avancées;
- Communiquer les progrès des clientes en organisant des réunions/évaluations
bihebdomadaires ; diffuser les résultats et les obstacles aux équipes des clientes;
- Recueillir des statistiques sur les clientes et les présenter à la directrice adjointe;
- Mettre à jour ses connaissances professionnelles en participant à des activités de
formation, en lisant des publications professionnelles, en entretenant des réseaux
personnels et en participant au programme HART;
- Démontrez de bonnes compétences en matière de documentation et dans la tenue des
dossiers
- Possédez des capacités d’organisation et de prise de décision
- Possédez d’excellentes compétences en matière de communication (verbale et écrite) et
de relations interpersonnelles
- Possédez d’excellentes compétences en matière de gestion de conflits, de résolution de
problèmes et d’esprit d’équipe
- Respectez les différences culturelles, les différences de statut socio-économique et la
diversité des modes de vie
- Avez la capacité à évaluer les situations de manière critique et objective, à prendre des
décisions réfléchies et à assurer le suivi
- Avez la capacité à gérer sainement le stress lié au travail
- Avez la capacité à travailler de manière flexible et autonome avec peu de supervision
- Êtes bilingue (français et anglais)
Qualifications:
- 3 ans d’expérience ou plus, de préférence dans le domaine des services sociaux
- Expérience professionnelle ou personnelle dans le soutien aux personnes ayant des
antécédents divers (déficience intellectuelle, troubles de santé mentale, toxicomanie,
etc.)
