The Change Management Specialist will be a member of the Central Intake Endoscopy regional team and play a key role in the successful implementation of the central intake service. The Change Management Specialist will work to establish/grow stakeholders’ awareness of the benefits of an electronic central intake referral management system and will support meaningful participation to ensure the local needs of clinicians and patients are incorporated into the process. The individual selected will be hands on, developing and nurturing relationships with clinicians and administrative support staff focusing on technical requirements.

**RESPONSIBILITIES**

**Technical Support**
- Initiate & lad stakeholder outreach and engagement activities with primary care providers and specialists, related to eReferral spread, onboarding in the region, to implementing new business processes at the office level
- Use knowledge base of the program to help clinicians and organizations understand the value coordinated access and eReferral can bring to their practices and patient experience
- Provide on-job support and formal training to primary care clinicians, specialists, and their support staff as needed
- Troubleshoot with Central Intake staff and participating hospitals to identify and resolve technical issues
- Document lessons learned and present findings in a logical and easy-to-understand manner
- Create and enable reinforcement mechanisms and celebrations of successes

**Data Quality**
- In conjunction with the Clinical Manager, develop an approach to ensure standardization of the data elements that will be captured/collected
- Provide on-job support and formal training to primary care clinicians, specialists, and their support staff as needed
- Work with the Ontario eServices program and the deployment teams to tailor readiness, on-boarding, and learning materials for local clinicians and organizations
- Identify potential risks, points of resistance, performance gaps and develop appropriate mitigation activities
- Establish standardized practice guidelines, and a quality assurance methodology to ensure the accuracy and timeliness of data entry and submission

**REQUIREMENTS**

**Education and Experience**
- Completion of post-secondary education (Degree or Diploma) in Health Sciences or Business Administration, or equivalent
- Solid experience in project/change management and/or health care in a relevant role including projects of a similar scale/scope preferred
- Previous experience with enabling technologies and digital health
- Experience providing data and analysis to support planning and decision making
- Well versed in use of virtual meeting tools (eg, Zoom, Teams), computers and basic troubleshooting in the work environment - familiarity with electronic medical record systems an asset
- Experience in healthcare evaluation and research an asset
- Change Management Professional certification is an asset
- Regulated Health Professional is an asset

**Knowledge**
- Thorough understanding of change management strategies and principles
- A solid understanding of clinical processes in primary care and specialist care
- Knowledge of the Ontario health care system and its institutions

**Skills and Abilities**
- Skilled at challenging accepted ideas and practices to promote consideration of new improved approaches
- Excellent relationship building, collaboration, and engagement skills, including conflict resolution, negotiation and mediation skills
- Ability to work effectively with a diverse group of clinical and non-clinical stakeholders in a variety of organizations
- Excellent oral and written communication skills in English; ability to effectively communicate in French will be considered an asset
- Demonstrated ability in developing and delivering training for primary care and specialists on the use of digital health technologies
- Analytical and systematic approach to problem-solving
- A demonstrated track record of being a change agent
- Occasional regional travels may be required
