**Caregiver Education Counsellor**
**The Role**

Reporting to the Client Services Manager - Community Programs, you will be responsible to deliver and continue to develop the caregiver support and education program, within an integrated wellness model, to support caregivers who are providing care to a senior 55 years or older.

Caregiver support services include: wellness education, caregiver counselling, support groups, information and referral services, including web-based resources, workshop development, co-ordination, and delivery, and aiming to maximize co-ordination amongst services, ensure access to services, and meet caregivers’ support needs. Services are provided in group or individual sessions on a time-limited basis. To support agency response to elder abuse and suicide prevention.

***Key responsibilities of the position**:

- Provide direct service to caregivers including assessment, wellness education, training, or information services, and maintain integrity of client records.
- Ongoing development of Caregiver Support and Education Services within the CHATS Wellness Framework, including the planning, implementation, and evaluation of the Program.
- Liaise and advocate both internally and externally with caregiver networks or community organizations to support and promote caregiver wellness and service integration, and profile CHATS.

**What you bring to the role**:

- Social Service Worker, SSW-G or related field.
- Current membership with the Ontario College of Social Workers and Social Service Workers is preferred.
- Minimum of 3 years’ experience providing support to older adults and/or caregiver support services in the community, including program development.
- Experience working within a multi-disciplinary team environment.
- Excellent assessment, interpersonal and problem-solving skills.
- Demonstrated presentation, teaching, and coaching skills.
- Excellent verbal and written communication skills.
- Ability to accommodate a flexible work schedule, including evenings and weekends.
- A valid Ontario drivers’ license and a vehicle are required.
- Current certification in First Aid and CPR.
- Fluency in both official languages (English or French) or other languages, an asset.
