Community Caregiver Wellness Coordinator 1 Position
**Full time permanent - *35 hrs/wk)**
The Caregiver Supports team provides an access point that responds to the growing and diverse needs of care partners in Scarborough and enables them to maintain the independent living status of their loved one in the community. The Caregiver Wellness Coordinator, working with other team members, will provide and support health and wellness education/programming, care connections and resource referrals internally and externally to programs and services for seniors, adults with disabilities and their care givers/partners. The position will be situated within the broader community support services portfolio.
**Reporting to**:Community Health and Support Services Manager
Purpose of the position:
The Caregiver Wellness Coordinator will develop, conduct and source culturally-appropriate health and wellness education/programming, engage in care and service coordination, as well as conduct outreach in a community-based setting. They will collaborate with the team to promote health and wellness and contribute to the holistic needs of the caregiver while supporting the Mission, Vision, and Values of Scarborough Center for Healthy Communities through their work.
**Key Areas of Responsibilities**:
- Provide services/programs on different platforms (virtual, in person) based on clients’ needs
- Design, deliver and facilitate educational as well as social health and wellness sessions based on identified caregiver needs.
- Outreach with community partners and internal programs to build new connections and foster referral pathways
- Process referrals and requests for services and support and connect clients to resources
- Collaborate with team members on an ongoing basis to support client health and social goals.
- Coordinate, schedule and track monthly phone contacts and face-to-face visits.
- Participate in the development and ongoing review and coordination of client identified goals and needs.
- Provide appropriate documentation for services and client support.
**Professional Qualifications**:
- Registered Practical Nurse or a degree in the field of community health promotion and/or gerontology is preferred
- Adult education or facilitation certification is also considered an asset
- CAMH Understanding Mental Health and Well-being in Later Life facilitator training considered an asset
**Level of Experience**:
- 2-5 years’ experience working in a community-based healthcare setting
- 1-3 years’ experience coordinating care
- Previous demonstrable experience developing and leading programs, workshops and/or adult education sessions
- Lived experience as a caregiver considered an asset
**Skills and Attributes**:
- Strong facilitation skills
- Ability to assess, plan, implement and evaluate quality client-centered care for individuals and various client populations.
- Experience working in a multi-cultural setting. Understand the community served - community connectedness.
- Strong digital literacy and documentation skills (Microsoft Office, PS Suits, Zoom).
- Ability to prioritize responsibilities and adapt to changing environments as necessary.
- Strong leadership abilities and experience supervising volunteers/student.
- Excellent organizational skills and the ability to multi-task
- Second language such as Urdu, Tamil, Hindi, Cantonese and Mandarin considered an asset
**Significant Working Conditions**:
- Flexibility of hours
- evenings and weekend work may be required
- Ability to lift and move 10-15 lbs.
- Ability to work from several organization locations due to program structure.
- Valid Drivers’ Licence to drive in Ontario and access to a vehicle would be considered an asset
- Subject to a Vulnerable Sector Screening Police Reference Check

Pay: $23.89-$26.98 per hour
Expected hours: 35 per week
