Reporting to the Manager, Student Rights & Responsibilities (SRRO), and receiving day-to-day supervision by the Case Management Specialist (CMS), the Case Management Facilitator (CMF) assists in assessing individual student needs and facilitating interventions to students experiencing barriers and/or demonstrating worrisome behaviour. The CMF provides lower-level case management and risk assessment for all aspects of Student at Risk Intervention Team "Care Team" cases, including at-risk behaviour, hospitalizations, prolonged absences, health and mental wellness, homelessness, poverty, abuse, sexual violence, and lack of basic needs. The CMF supports the triage and risk assessment process by working with the CMS through the early identification of students demonstrating concerning or worrisome behaviour and supports the CMS in facilitating student supports. The CMF supports recommended interventions, agreements, and strategies for success and maintains all documentation and information utilizing the case management database. The CMF participates in follow-up appointments, as directed, with students to monitor and assess progress and flags any concerns to the CMS. The CMF uses established case management frameworks and best practices to assist with developing and implementing programs, communication initiatives, and training for Sheridan faculty and staff to promote awareness of student supports and provides tools on how to proactively recognize and support students demonstrating concerning or worrisome behaviour. In conjunction with the CMS, the CMF serves as a point of contact for information and referrals for unique, sensitive and more complex situations that require coordinated support.

What You’ll be Doing

Providing case management support to complex, unique, and sensitive student cases for students experiencing barriers and/or demonstrating worrisome behaviour; including assisting the CMS with intake, triage, assessment, providing coordinated care and support through consultation with internal and external partners, developing individual support plans, monitoring, and following up on progress and transition planning;
Participating as a member of a multi-disciplinary Care team of 10+members through a structured process of assessing individual student needs and providing coordinated interventions to students experiencing barriers and/or demonstrating worrisome behaviour. Supports CMS in organizing Care Team meetings, presents case updates and takes notes;
Taking highly sensitive, and confidential case notes documenting all actions, conversations, interventions, implementation plans, progress reports, and follow-ups within the Advocate Case Management database while ensuring proper record-keeping and file retention is in accordance with best-practice standards and based on FIPPA and PHIPPA guidelines;
Participating with institutional campus-wide internal committees/working groups (i.e Sexual Violence Taskforce) as well as external community agencies (i.e. Peel Situational Table, and Halton Transitional Youth Committee) to support students’ wellbeing as needed and directed by the Manger, Students Rights and Responsibilities (MSRRO). Supports the awareness, education, communication strategies, and marketing/educational material as directed by the CMS and delivers training and education sessions on student health, mental health, and success;
Participating in other projects and activities as assigned including support for new student orientation and Student Affairs initiatives as well as other special projects and research assigned by the Manager, SRRO;
Performing other related duties, as assigned.

3-year degree/diploma in Social Work, Social Sciences, Psychology or other related field

2 years of experience with case management, including cases related to mental health, and individuals experiencing socioeconomic barriers, preferably in a post
